Doors to an Air-Lock should be self-closing and without any hold back arrangements. Vessel has an Air-Lock on the motor room entrance. PSCOs observed the absence of any self-closing arrangements on the outer Air-lock door. In addition, a hold back arrangement was identified on the exterior bulkhead of the motor room. PSCOs observed a certified safe crew notification light on top of the cargo compressor room phone booth, that appeared to not be gas-tight. Improper cable gland with rubber gasket was installed. Vessel required to have LRIT technician attend and attest to operation of LRIT. Vessel required to have classification society attend and attest to technician report, proper re-installatin of LRIT antenna, and verify distance between keel and highest point of vessel. 74SOLAS(14)V/19-1.4.2
