A pre-engineered fixed gas fire extinguishing system must: (1) Be approved by the Commandant; (2) Be capable of manual actuation from outside the space in addition to automatic actuation by a heat detector; (3) Automatically shut down all power ventilation systems and all engines that draw intake air from within the protected space; and (4) Be installed in accordance with the manufacturer's instructions. MI observed an incompletely installed pre-engineered fixed gas fire extinguishing system located in the engine space. Complete installation and provide a technician report attesting to the satisfactory operation of the system. Complete prior to the carriage of passengers. 46 CFR 181.420 (a)

A durable placard must be posted next to all radiotelephone installations with the emergency broadcast instructions and information, specific to the individual vessel. MI observed no placard posted onboard. Post satisfactory placard to the satisfaction of attending [name withheld] within two weeks. 46 CFR 184.506

The master and crew of a vessel will be familiar with the content of and have mounted at the operating station, emergency instructions containing the actions to be taken in the event of fire, heavy weather, or man overboard conditions. MI observed no emergency instructions mounted at the operating station. Mount satisfactory instructions to the satisfaction of the attending [name withheld] within two weeks. 46 CFR 185.510

A ship, except a ship of less than 26 feet in length, must have a placard of at least 5 by 8 inches, made of durable material fixed in a conspicuous place in each machinery space, or at the bilge and ballast pump control station, stating the information written in 46 CFR 155.450 (a). MI observed no placard mounted onboard. Mount placard within two weeks to the satisfaction of the attending MI. 46 CFR 155.450 (a)

Every vessel subject to Part III of Title III of the Communications Act must have a detailed inspection of the radio installation by an FCC-licensed technician in accordance with § 80.59 once every five years. The FCC-licensed technician must use the latest FCC Information Bulletin, How to Conduct an Inspection of a Small Passenger Vessel. If the ship passes the inspection, the technician will issue a Communications Act Safety Radiotelephony Certificate. Vessel does not have a Communications Act Safety Radiotelephony Certificate. Provide proof of a valid Communications Act Safety Radiotelephony Certificate to the attending MI within two weeks. 47 CFR 80.901

The master of the vessel shall ensure that: The EPIRB's battery is replaced after it is used, or before the date required by FCC regulations in 47 CFR part 80, whichever comes sooner. Inspector observed that the batteries for the EPIRB were expired. 46 CFR 185.728(b) 701c

Each item of lifesaving equipment determined by the marine inspector to not be in serviceable condition must be repaired or replaced. MI observed life jackets beginning to crack. Recommend repair or replace. 46 CFR 176.808(b)

On a vessel of at least 26 feet in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in a machinery space bilge. MI observed machinery space bilge high level audible and visual alarms not operating as intended. 46 CFR 182.530(a)

The alcohol testing of each individual must be conducted within 2 hours of when the SMI occurred, unless precluded by safety concerns directly related to the incident. MI observed expired alcohol test swabs. 46 CFR 4.06(a)

A vessel to which this subchapter applies may not be operated without having on board a valid U.S. Coast Guard Certificate of Inspection. Vessel was due for renewal COI inspection on 11JAN24 and has yet to receive a renewal COI inspection. Vessel shall not operate with passengers until new COI has been issued. 46 CFR 176.100

All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules. MI found the starboard navigation light extinguished. The vessel is limited to daylight without reduced visibility operations. 46 CFR 183.420

On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level. The forward high water alarm did not have a visual indicator at the operating station. 46 CFR 182.530(a)

A hatch exposed to the weather must be watertight. MI found the gasket around the forward-most hatch to not be watertight. Gasket was hanging off the hatch when opened. 46 CFR 179.360 (a)

Placards containing instructions for the donning and use of the life jackets aboard the vessel must be posted in conspicuous places that are regularly accessible and visible to the crew and passengers. MI found placard did not give instructions for how to properly don the model of life jackets aboard. 46 CFR 185.516 (a)

As appropriate for the intended voyage, a vessel must carry adequate charts, U.S. Coast Pilot or similar publication, Coast Guard Light List, and Tide and Current Tables. MI found Coast Pilot [name withheld], and tide and Current Tables to be outdated. 46 CFR 184.420

The items contained in first-aid kit shall be as listed in Table 160.041-4(b). MI found inadequate amount of aspirin. Owner/Operator obtained 100 capsules of aspirin and placed aboard in first-aid kit. 46 CFR 160.041-4(b)

If posting is impracticable, the stability letter must be kept on board in a weather tight container readily available for use by the crew and display to passengers and others on request. MI found vessel's stability letter was not on board. Owner/Operator produced stability letter and showed proof to MI. 46 CFR 176.306

A vessel must comply with the applicable requirements for any radio and electronic position indicating radio beacon. Vessel has wrong MMSI number programmed into radio. Program correct MMSI number into radio.

A vessel must carry either a first aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. Ammonia inhalants are missing from first aid kit. Provide ammonia inhalants for first aid kit.

01. Procure and install a new 40:BC fire extinguisher.

02. Make forward hatch covers water tight.
