46 CFR 185.740(b) Each disposable hydrostatic release unit must be marked with an expiration date of two years after the date on which the unit was installed. -Hydrostatic release had expired.

A certification that the ship has passed an inspection must be entered into the ship's log by the inspecting technician. The technician conducting the inspection and providing the certification must not be the vessel's owner, operator, master, or employee or their affiliates. Additionally, the vessel owner, operator, or ship's master must certify in the station log that the inspection was satisfactory. There are no FCC prior notice requirements for any inspection pursuant to paragraph (a)(1) of this section. An inspection of the bridge-to-bridge radio stations on board vessels subject to the Vessel Bridge-to-Bridge Radiotelephone Act must be conducted by the same FCC-licensed technician. Vessel had an expired FCC Radio Safety Certificate.

The marine employer must have a sufficient number of alcohol testing devices readily accessible on board the vessel to determine the presence of alcohol in the system of each individual who was directly involved in the SMI. No alcohol testing devices onboard.

A pre-engineered fixed gas fire extinguishing system must: Automatically shut down all power ventilation systems and all engines that draw intake air from within the protected space. Power ventilation & engines did not automatically shut down under simulated system activation.

The Certificate of Inspection and any SOLAS Certificates must be posted under glass or other suitable transparent material, such that all pages are visible, in a conspicuous place on the vessel where observation by passengers is likely. If posting is impracticable, such as in an open boat, the certificates must be kept on board in a weathertight container readily available for use by the crew and display to passengers and others on request. A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked “First-Aid Kit”. A first-aid kit must be easily visible and readily available to the crew. Aspirin in first aid kit expired.

Each life jacket, immersion suit, and ring life buoy must be marked in clearly legible block capital letters with the vessel's name. The marking is not required on a life jacket carried to meet a temporary need for additional life jackets, if the life jacket has the name of another vessel or company marked on it. For an immersion suit, the name of the person to whom the immersion suit is assigned is an acceptable alternative to the name of the vessel. Life buoy missing vessel name.

A vessel not required to have a power driven fire pump by § 181.300 must have at least three 9.5 liter (21/2 gallon) buckets, with an attached lanyard satisfactory to the cognizant OCMI, placed so as to be easily available during an emergency. The words “FIRE BUCKET” must be stenciled in a contrasting color on each bucket. Vessel had 02 5gal buckets onboard.

Every vessel subject to Title III of the Communications Act must have a detailed inspection of the radio installation by a licensed technician and issued a Safety Radio Certificate. Vessels Safety Radio Certificate expired. 47 CFR 80.901

All vessels must have navigation lights that are in compliance with the sections of the Inland Navigation Rules. Vessels Starboard navigation light was inoperable. 46 CFR 183.420

Vessel must carry a first aid kit approved IAW 46 CFR 160.041. Vessels first aid kit had missing and expired conents. 46 CFR 184.710

A Vessel on which a pre-engineered fixed gas fire extinguishing system is installed must have at the operating station a light to indicate discharge and an audible alarm tha tsounds. Fire boy alarm was not audible. 46 CFR 181.420

Each ring buoy must be marked with Type II retro-reflective material. Vessels ring buoy retro material not of approved type. 46 CFR 185.604

Prove proper operation of the engine room’s high water indicating light located at the OP Station console. Note: Audible alarm worked but light didn't work.
