TO ASSIST IN CASUALTY INVESTIGATION SHIPS, WHEN ENGAGED ON INTERNATIONAL VOYAGES, SUBJECT TO THE PROVISIONS OD REGULATION 1.4 SHALL BE FITTED WITH A VOYAGE DATA RECORDER AS FOLLOWS: SHIPS OTHER THEN PASSENGER SHIPS OF 3,000GT AND UPWARDS CONSTRUCTED ON OR AFTER 01JUL2002. VDE WAS INOPERABLE AT TIME OF INSPECTION.
Condition: Improper/Lack of Maintenance
Action required: 60 - Rectify deficiencies prior to movement
Resolved 5 April 2022
Resolution: Radio inspection company attended ship for inspection and repair. Several function tests were performed to check VDR status message and battery performance test. All found in good order.

FALLS USED IN LAUNCHING SHALL BE INSPECTED PERIODICALLY WITH SPECIAL REGARD FOR AREAS PASSING THROUGH SHEAVES, AND RENEWED WHEN NECESSARY DUR TO DETERIORATION OF THE FALLS OR AT INTERVALS OF NOT MORE THEN 5 YEARS WHICHEVER IS EARLIER. RESCUR BOAT LAUNCH DAVIT CABLES LAST RENEWED MARCH 2017 AND HAS EXPIRED.

Resolved 5 April 2022
Resolution: New replacement Rescue boat and new davit cables complying with the required specifications of the ship builder were supplied and fitted. Rescue boat and launching appliances were tested with satisfactory results.
