Rails or equivalent protection must be installed near the periphery of all decks accessible to crew. Equivalent protection may include lifelines, wire rope, chains, and bulwarks that provide strength and support equivalent to fixed rails. The entire periphery of the stern area of the vessel has a raised grating over the steering gear that significantly reduces the height of the gunwale offering no fall overboard prevention. Install appropriate railing at the stern of the vessel to the satisfaction of a CG marine inspector.

Each towing vessel certificated under the provisions of this subchapter must be in full compliance with the terms of the COI. The vessel's current COI has a condition that reads as follows: "LBS...Also, in fair weather only, vessel is authorized to transit within the marked channel of the Atchafalaya River not to exceed five (5) nautical miles beyond Atchafalaya Channel lighted bouys 1 and 2. Vessel must have a minimum of one (1) rigid buoyant apparatus, one (1) EPIRB and six (6) night distress signals for this transit. Decisions regarding weather shall be made in accordance with company safety management system and/or policy provided to the Master." There is no company safety management system nor a policy provided to the master IAW the condition. Provide either a valid company policy or safety management system for this route, to this office for review.

Fuel hose, if designed for use with clamps, must be installed with two hose clamps at each end of the hose. Some of the fuel hoses over barbed fittings on the generators were not double-hose-clamped. Install additional hose clamps on all fuel hoses or otherwise make hoses comply with 46 CFR 143.265(d)(2)(v).

Each towing vessel must have a reliable means to provide notification of high bilge levels. High bilge level alarm was missing from forward hold. Install high bilge level alarm in forward hold. Due date was changed from "Next dry dock" to "30 days". Due date is extended to 21 Jan 2024 to allow full 30 days for operator to make change.
