46 CFR 111.105-31 (m) Locations in the weather are Class I, Division 1 (Zone 1) locations may have only approved intrinsically safe, explosion proof, or purged and pressurized electrical equipment, and through runs of marine shipboard cable. MI observed multiple cable glands on electrical equipment in the hazardous zone that had failed which compromises intrinsically safe, explosion proof capabilities. Identified locations are small gland on vacuum tank, temps sensors on P/S slop tanks, 7 P/S ballast sensors, 7 P/S cargo gauge point and temp indicators, 6 P/S cargo level indicators, and 5 Port cargo temp. Cable gland shall be repaired to the satisfaction of USCG Marine Inspector.

46 CFR 162.017-4 Pressure-vacuum relief valves may be inspected and tested at the plant of the manufacturer. An inspector may conduct such tests and examinations as may be necessary to determine compliance with this specification. Vessel was unable to provide proof of testing during last dry dock. Vessel shall provide proof of P/V valve testing.

46 CFR 35.35-70 (c) Cargo pump pressure gage shall be tested at least once a year for accuracy. MI observed that it has been over a year since gages have been tested. Vessel shall test gages and provide proof to USCG Marine Inspector.

46 CFR 35.35-70 (b) Cargo pump relief valves shall be tested at least once each year to determine that they function satisfactorily at the pressure at which they are set to open. MI observed that it has been over a year since cargo pump relief valves have been tested. Vessel shall test relief valves and provide proof to USCG Marine Inspector. CODE 705: Vessel provided an approved procedure, and risk assessment for testing cargo pump relief valves with cargo. Vessel requested to be allowed to load cargo and complete testing before discharge. Request is approved. Vessel shall provide proof of testing before discharge.

33 CFR 155.310 (b) (3) A mechanical means of closing each drain and scupper within the coaming. Identified that all scupper plugs were missing within the spill containment both port and stbd side. Install plugs before cargo operations.

46 CFR 31.10-15(b) The inspection shall be such as to insure that the vessel, as regards the structure, and other equipment is in satisfactory condition and fit for the service for which it is intended. MI observed that PCO room, both thermal oil room, #1 port ballast, and #2 port ballast watertight doors were not sealing, and daylight was observed between knife edge and gasket when doors were dogged shut. Enact repairs to the satisfaction of USCG Marine Inspector.

46 CFR 31.10-15(b) The inspection shall be such as to insure that the vessel, as regards the structure, and other equipment is in satisfactory condition and fit for the service for which it is intended. MI observed that grating by #1 port forward ballast tank hatch was not properly secured. Vessel shall secure grating.

46 CFR 111.01(a)(2) Electrical installations on vessels must ensure protection of vessel from electrical hazards. MI observed three areas where wiring conduit corroded which will start chaffing wires. Located stbd side bow main deck behind life raft, and the three light fixtures port and stbd side above manifolds. Repair to the satisfaction of USCG Marine Inspector.

46 CFR 58.10-5 (a) Engine design. All installations must be of marine type engines suitable for the intended service, designed and constructed in conformance with the requirements of this subchapter. MI was informed that stbd generator fuel pump failed and that metal shaving were sent through oil lubrication system. Vessel submitted risk mitigation plan attesting to being able to safely conducted cargo operations at 40% capacity on one generator. CODE 705: Vessel approved to conduct cargo operations at 40% capacity until stbd generator is repaired.

46 CFR 31.10-15 The inspection shall be such as to insure that the vessel, as regards the piping, main and auxiliary machinery, and other equipment is in satisfactory condition and fit for the service for which it is intended. MI identified a fuel leak on port generator fuel supply line.

46 CFR 34.15-10 (h) Complete but simple instructions for the operation of the systems must be located in a conspicuous place at or near all pull boxes. MI observed missing label on one of the therm oil heater CO2 activation handles outside the space. Replaced label.

46 CFR 34.10-5 (e) Fire pumps shall be fitted with a pressure gage on the discharge side of the pumps. MI observed that stbd fire pump didn't have a gage. Gage was installed.

The support for all cables should be such to prevent undue sag, but in no case should exceed the distance between frames or 610mm (24in), whichever is less. The cable supports over the cargo deck are wasted. Make temporary repairs to areas of the cable run over the cargo deck that are wasted greater than 24" within 14 days. 46 CFR 31.35 46 CFR 110.10-1(j)(5) IEEE Std. 45-1998 10.5

No extensive repairs or alterations, except in emergency, shall be made to any fire-extinguishing apparatus, or other appliance subject to inspection, without advance notice to the Officer in Charge, Marine Inspection. Such repairs or alterations shall so far as is practicable be made with materials and tested in the manner specified within the regulations in this subchapter and subchapter Q (Specifications) of this chapter for new construction. Section of CO2 system piping was unintentionally charged, vessel shall have technician make sufficient repairs and provide report to OCMI. 46 CFR 35.01-35(a)

Boilers, main and auxiliary machinery, and piping systems shall conform to the requirements of subchapter F (Marine Engineering). Port generator room was missing the battery disconnect switch. 46CFR32.35-1

The installation of all electrical engineering or interior communications systems, together with the details of design, construction, and installation, must meet the requirements of subchapter J (Electrical Engineering Regulations). Room heater cables located in inert gas room were out of the junction box. 46CFR32.45-1

Provide proof of testing for air receiver relief valves (02).

Each transfer pipe system must not leak under static liquid pressure at least 1 1/2 times the maximum allowable working pressure. The last test of the vessel's cargo piping system was over a year ago. The vessel must conduct an annual hydro-static test of its cargo piping.

Deck foam systems shall have a sample of the foam liquid submitted for analysis. Vessel did submit a sample, but did not have a report to provide. Vessel is to provide an analysis report when made available.
