The technical and or operational nature of deficiencies #2-8, #10, #12, and #14 provides objective evidence of a serious failure of the ISM code. PSCO observed multiple explosion proof lights in the cargo pump room missing glands from removed cable runs or missing ground wires. Additionally, PSCO observed explosion proof lights on either side of the pump room entrance missing ground wires and 03 explosion proof lights on the forward superstructure improperly grounded or with wires not properly fitted. Finally, PSCO observed an explosion proof light in the port deck locker with a compromised gland and a light outside the starboard deck locker that is not rated for use in a hazardous area.

PSCO observed “A” class self-closing galley inner door not properly latching as well as a hold back hooks fitted on both galley outer doors. Additionally, doors located in the main stairwell including: an “A” class self-closing fire door to pilot cabin was missing self-closing device, the navigation bridge toilet door did not properly latch, and the “D” deck door is not properly self-closing. These doors are indicated as self closing per vessel's fire control plan.

A firefighter's outfit shall consist of a set of personal equipment and a breathing apparatus. Protective clothing of a material to protect skin from the heat radiating from the fire and from burns and scalding by steam. The outer surface shall be water resistant.
Breathing apparatus shall be self-contained compressed air breathing apparatus for which the volume of air contained in the cylinder shall be at least 1200 liters or other self-contained breathing apparatus which shall be capable of functioning for at least 30 minutes all cylinders for breathing apparatus shall be interchangeable.
PSCO observed exposed skin on all four members of the fire team, and one member of the fire team improperly donned the SCBA mask improperly affixing the regulator.

PSCO observed crew using cargo to test cargo pump room fixed gas detection system due to low quantities of span gas on board. Additionally, PSCO observed the sample line for the #4 void (line 21) was disconnected from the fixed gas detection and instead sampling the “spare” (line 22), which was drawing a sample from CCR. These sampling hoses were swapped and incorrectly identified on the ship's sample point list.
