In meeting these requirements, the company should ensure that: inspections are held at appropriate intervals; any non-conformity is reported, with its possible cause, if unknown; appropriate corrective action is taken; and records of these activities are maintained. The company should establish procedures to ensure that ship is maintained in accordance with the provisions of the relevant rules and regulations established by the company. The technical and or operational nature of deficiencies #2-8, #10, #12, and #14 provides objective evidence of a serious failure of the ISM code. An external audit report satisfactory to the OCMI must be submitted prior to clearance of this deficiency. 74 SOLAS (20)IX/3.1 ISM CODE 10.2

No electrical equipment shall be installed in any space where flammable mixtures are liable to collect including those on-board tankers or in compartments assigned principally to accumulate batteries, in paint lockers, acetylene stores or similar spaces. PSCO observed multiple explosion proof lights in the cargo pump room missing glands from removed cable runs or missing ground wires. Additionally, PSCO observed explosion proof lights on either side of the pump room entrance missing ground wires and 03 explosion proof lights on the forward superstructure improperly grounded or with wires not properly fitted. Finally, PSCO observed an explosion proof light in the port deck locker with a compromised gland and a light outside the starboard deck locker that is not rated for use in a hazardous area. 74 SOLAS (2004) II-1/45.10.2

A secondary means of allowing full flow relief of vapour, air, or inert gas mixtures shall be provided to prevent over pressure or under pressure in the event of failure. Alternatively pressure sensors may be fitted in each tank protected by the arrangement required in paragraph 6.1.2, with a monitoring system in the ship's cargo control room or in the position from which cargo operations are normally carried out such monitoring equipment shall also provide an alarm facility which is activated by the detection of overpressure or under pressure conditions within a tank. PSCO observed cargo tank pressure sensing alarms inoperable. Vessel is not fitted with a mast riser. 74 SOLAS (04) II-2/ 11.6.3.2

The high-level alarm required must activate an alarm once the cargo level reaches 95% of the tank capacity or higher but before the tank overfill alarm. PSCO observed inoperable high-level alarms due to inoperable tank radar system. 46 CFR 39.2007(c)(1)

The fire resistance of doors shall be equivalent to that of the division in which they are fitted this being determined in accordance with the fire test procedures code. PSCO observed “A” class self-closing galley inner door not properly latching as well as a hold back hooks fitted on both galley outer doors. Additionally, doors located in the main stairwell including: an “A” class self-closing fire door to pilot cabin was missing self-closing device, the navigation bridge toilet door did not properly latch, and the “D” deck door is not properly self-closing. These doors are indicated as self closing per vessel's fire control plan. 74 SOLAS (04) II-2/ 9.4.2.1

Electrical installations shall be such that the safety of the passengers, crew, and ship from electrical hazards will be ensured. PSCO Observed An electrical panel in the D deck electrical duct missing the cover, dead in runs in both officer and crew laundry, an unapproved splice on an electrical load in officer laundry, multiple ungrounded light fixtures around the superstructure, and a 220 Volt low insulation alarm on the emergency switchboard. 74 SOLAS (04) II-1/ 40.1.3

The purpose of this regulation is to prevent the ignition of combustible materials or flammable liquids for this purpose the following functional requirements shall be met; Means shall be provided to control leaks of flammable liquids. PSCO Observed oil leaks under the main engine cylinder heads #1 and #2, an active leak on the overhead piping in the purifier room, and oil accumulation under the emergency generator fuel tank and under the IGG fuel pump. 74 SOLAS (04) II-2/ 4.1.1

A firefighter's outfit shall consist of a set of personal equipment and a breathing apparatus. Protective clothing of a material to protect skin from the heat radiating from the fire and from burns and scalding by steam. The outer surface shall be water resistant. Breathing apparatus shall be self-contained compressed air breathing apparatus for which the volume of air contained in the cylinder shall be at least 1200 liters or other self-contained breathing apparatus which shall be capable of functioning for at least 30 minutes all cylinders for breathing apparatus shall be interchangeable. PSCO observed exposed skin on all four members of the fire team, and one member of the fire team improperly donned the SCBA mask improperly affixing the regulator. 74 SOLAS (20) II-2/ 10.1.1 FSS Code 3.2.1.1 FSS Code 3.2.1.2.1

The ring, lines, and letters shall be painted in white or yellow on a dark ground or in black on a light ground they shall also be permanently marked on the sides of the ship to the satisfaction of the administration. The marks shall be plainly visible and if necessary, in special arrangements shall be made for this purpose. PSCO observed port side load line with lack of paint making the active load line indistinguishable from the other existing load lines. ICLL Annex 1 – Reg 8

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to see without danger to the ship or persons on board. PSCO observed crew using cargo to test cargo pump room fixed gas detection system due to low quantities of span gas on board. Additionally, PSCO observed the sample line for the #4 void (line 21) was disconnected from the fixed gas detection and instead sampling the “spare” (line 22), which was drawing a sample from CCR. These sampling hoses were swapped and incorrectly identified on the ship's sample point list. 74 SOLAS (2020) I/ 11(a)

The last 1.0m of vapor piping upstream of the vessels vapor connection and each end of the vapor hose must be painted in the sequence of red/yellow/red with the width of the red bands must be 0.1m and the width of the middle yellow band must be 0.8m. PSCO Observed all four vapor manifolds improperly painted. 46CFR 39.2001(h)

Every oil tanker of 150 gross tonnage and above shall carry on board a shipboard oil pollution emergency plan approved by the administration. PSCO observed vessel has a SMPEP. IOPP Supplement Form B indicates vessel has a SOPEP. Vessel does not have an IPPC-NLS certificate or Certificate of Fitness for the carriage of NLS. MARPOL I/ 37.1
