In multi-engine installations which are supplied from the same fuel source, means of isolation the fuel supply and spill piping to individual engines shall be provided. The means of isolation shall not affect the operation of the other engines and shall be operable from a position not rendered inaccessible by a fire on any of the engines. The fuel oil supply valve for the #2 Auxiliary Engine contained two pieces of metal that were inserted beside the stem preventing the valve from closing from a remote location. Guard rails fitted on superstructure and freeboard decks shall have at least three courses. The opening below the lowest course of the guard rail shall not exceed 230mm. The other courses shall be not more that 380mm apart. In the case of ships with rounded gunwales the guard rail supports shall not be place flat on the deck. In other locations guard rails with at least two courses shall be fitted. Guard rails shall comply with the following provision at least every third stanchion shall be supported by a bracket or stay.
