Each towing vessel must perform the applicable tests in Table 143.245(b) of this section. The tests required by this section must be recorded in accordance with part 140 of this subchapter. Such tests include essential systems at least once every 3 months. Emergency de-watering or ballast management is considered an essential system. Marine Inspector observed that the vessel had an installed bilge system for emergency de-watering but has been unable to test it. Take appropriate action to either test the bilge system or blank off the bilge system to the satisfaction of the OCMI.

Each towing vessel must be fitted with a general alarm that is capable of notifying persons in any accommodation, work space, and the engine room. General alarm was not heard in all required spaces. Repair/replace to the satisfaction of the USCG Marine Inspector.

Vessel did not have record of testing alarm set points. Test and record alarm set points to the satisfaction of the USCG Marine Inspector.

Each operator or agent of a vessel with a capacity of 250 or more barrels of fuel oil, cargo oil, hazardous material, or liquefied gas as regulated in Table 4 of 46 CFR part 154, or each person who arranges for and hires a person to be in charge of a transfer of fuel oil, of a transfer of liquid cargo in bulk, or of cargo-tank cleaning, shall designate, either by name or by position in the crew, the person in charge (PIC) of each transfer to or from the vessel and of each tank-cleaning. Vessel was found to be conducting fuel oil transfers with the dredge OREGON which does not currently comply with required transfer personnel, procedures, equipment, or records. ITV WE IVANOFF is restricted from receiving fuel from the dredge OREGON until the dredge is in compliance with 33 CFR 155 to the Marine Inspectors satisfaction.

Means must be provided for stopping each fan in a ventilation system serving machinery spaces and for closing, in case of fire, each doorway, ventilator, and annular space around funnels and other openings into such spaces. Vent closures found to be inoperable. Repair and replace to ensure vent capable of closing. 46 CFR 144.605

The following list of items must be recorded in the TVR, official logbook, or in accordance with the TSMS applicable to the vessel: (1) Personnel records, in accordance with § 140.400; (2) Safety orientation, in accordance with § 140.410; (3) Record of drills and instruction, in accordance with § 140.420; (4) Examinations and tests, in accordance with § 140.615; (5) Operative navigational safety equipment, in accordance with § 140.620; (6) Navigation assessment, in accordance with § 140.635; (7) Navigation safety training, in accordance with § 140.645; (8) Oil residue discharges and disposals, in accordance with § 140.820; and (10) Fire-detection and fixed fire-extinguishing, in accordance with § 142.240. Vessel's logbook does not contain required entries. Create new logbook that incorporates required entries.

The master must ensure that all hatches, doors, and other openings designed to be watertight or weather-tight function properly. Stern hatches are no longer watertight and allow water ingress. Repair/replace to satisfaction of CG marine inspector.

Each exhaust pipe from an internal combustion engine which is within reach of personnel must be insulated or otherwise guarded to prevent burns. Exhaust piping in vicinity of operating station is not insulated or shielded. Install insulation/shield to prevent burns.

Lifejackets must have the following attachments and fittings: (1) Each lifejacket must have a lifejacket light approved under approval series 161.012 or 161.112 securely attached to the front shoulder area of the lifejacket. (2) Each lifejacket must have a whistle firmly secured by a cord to the lifejacket. Several lifejackets onboard did not have functioning lights/whistles and were not labeled with vessel name. Provide four fully outfitted lifejackets onboard as required by the COI.

At least two lifebuoys on a towing vessel greater than 26 feet must be fitted with a floating electric water light approved under approval series 161.010 or 161.110, unless the towing vessel is limited to daytime operation, in which case no floating electric water light is required. Starboard side lifebuoy light was found inoperable.

All owners and operators of an existing towing vessel must have a valid COI prior to the specified phase-in period. This vessel is one of the 50% of the vessels that has not obtained a valid COI. The vessel shall be made available for a Coast Guard COI inspection within 90 days. If an inspection is not completed prior to this date, the action for this deficiency shall be changed to a 60 (Rectify deficiencies prior to movement) and the vessel may no longer be permitted to operate. 46 CFR §136.202(a)

The fire-detection system design and installation must be certified and inspected by a registered professional engineer, NICET level IV fire alarm engineering technician or authorized classification society. The vessel's fire-detection system does not meet requirements in 46 CFR 142.330.

The alarms for the machinery and electrical systems must be visible and audible at each operating station. The audible alarm at the main operating station was inadequate for the existing noise levels in the pilot house and no alarms were installed on the operating station located outside the pilot house.

Marine employer must ensure that they and their crew members meet the Chemical Testing requirements of 46 CFR 16. Vessel was unable to provide documentation attesting to crew members enrolled in a drug and alcohol program.

The portable fire pump must have a discharge pressure gauge installed.

Vessel's HAZMAT cabinet must be secured to the vessel so that it does not move.
