The system should be capable of covering all areas mentioned in 11.3.1 with uniformly distributed water-spray.
PSCO observed both port and starboard deck nozzles and starboard manifold nozzles in multiple spots clogged and not spraying water.

During cargo deck walk, PSCO found broken grounding wires on the #1 starboard cargo tank pressure temperature panel and #2 and #3 cargo tank forward pressure temperature panel.

The vessel experienced an equipment failure while transiting the navigable waters of the United States which adversely affected its maneuverability and posed a hazard to vessels and infrastructure along the Houston Ship Channel.
Submit for review a Classification Society report to [email withheld] attesting to the cause of the equipment failure, that authorized temporary or permanent repairs have been completed, and that the vessel's full and normal maneuverability has been restored. 74SOLAS(04) II-I/26.1

Resolution: Reviewed and accepted class report from DNV-GL which stated the root cause was traced to the ME exhaust valve #3 improper closing. The exhaust valve was replaced with a spare kept on board.
1138- Liquefied Gasses in Bulk –Electrical installations should be such as to minimize the risk of fire and explosion from flammable products. While conducting a deck walk, PSCOs discovered 01 electrical light on deck was missing the proper plug where the cable feed through was not in use. The light had only the caplet glands in place.
