The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons onboard. PSCO observed Lube oil leak coming from number auxiliary generator. 74 SOLAS (20) I/11(a) Code 17abc

The Company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirement which may be established by the company. Due to the objective evidence in deficiencies 2, 3, and 5 the vessel is not in substantial compliance with relevant conventions, calling into question the adequacy and implementation of the vessel’s SMS under the ISM code. PSCO’s collected objective evidence that the vessel has an active uncontrolled fuel oil leak in the engine room since 12MAR2025, a leak in the main engine water cooling system, and quick closing valve for Emergency Diesel Generator has not been serviced, maintained, or operated. SOLAS 74 (2020 Cons.) IX/3.1 ISM Code Part A/10.1

The purpose of this regulation is to prevent the ignition of combustible materials or flammable liquids... Means shall be provided to control leaks of flammable liquids. PSCO’s observed fuel oil actively leaking from service tank pipe to main engine, resulting in approximately 8-12 inches of standing diesel fuel with multiple oil-soaked rags, were discovered in the bilge with no means of control and actively leaking since 12MAR2025. Vessel maintenance folder was reviewed by PSCO's and was discovered and confirmed by the chief engineer that the active oil leak was present starting 12MAR2025. SOLAS 74 (2004 Cons.) II-2 4.1.1

Oil fuel pipes, which if damaged, would allow oil to escape from a storage, settling, or daily service tank having a capacity of 500l and above.. Shall be fitted with a valve directly on the tank capable of being closed from a safe position outside the space concerned in the event of a fire. The fuel/oil quick closing valve for the emergency diesel generator was stuck in position and not able to be activated from outside the space. PSCOs observed the presence of excessive rust as well as the valve being painted over. SOLAS 74 (2004 Cons.) II-2 4.2.2.3.4

Bilge wells in... shall be located and monitored in such a way that the accumulation of liquids is detected at normal angles of trim and heel. PSCO’s observed that both audible and visual alarms for all high water bilge alarms were inoperable. SOLAS 74 (2004 Cons.) II-1 48

The machinery, boilers and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended. PSCO’s observed fracture in main engine cooling line causing a constant, steady flow of water. SOLAS 74 (2004 Cons.) II-1 26

The public address system shall be a loudspeaker installation enabling the broadcast of messages into all spaces where crew members or passengers, or both, are normally present, and to muster station. Public address system did not function internally on the poop deck. 74SOLAS(20) III/6.4.2, LSA 7.2.2.1

Cargo ships shall carry at least one rescue boat complying with the requirements of section 5.1 of the code. Rescue boats shall have sufficient mobility and manoeurvability in a seaway to enable persons to be retrieved from the water. Rescue boat engine will not remain at idle without constant throttle input. 74SOLAS(20) III/31.2, LSA 5.1.1.7
