Whenever damage or deterioration to hull plating or structural members that may affect the seaworthiness of a vessel is discovered or suspected, the cognizant OCMI may conduct an internal structural examination in any affected space including fuel tanks, and may require the vessel to be dry-docked or taken out of service to assess the extent of the damage, and to effect permanent repairs. Vessel ran aground over the eastern rock dike on Fresh Bayou Canal near mile marker 14 while transiting southbound. essel shall submit a salvage plan to the local OCMI for approval prior to commencing salvage operations. Vessel shall submit a transit plan to the local OCMI for approval prior to transiting to the shipyard. Vessel shall be subject to a drydock examination by the local OCMI prior to departure.

Immediately after the addressing of resultant safety concerns, the owner, agent, master, operator, or person in charge, shall notify the nearest Sector Office, Marine Inspection Office or Coast Guard Group Office whenever a vessel is involved in a marine casualty. Vessel was involved in an unintended grounding on September 7th, 2024 at approximately 0230 hours (local time) on Fresh Water Bayou near mile marker 14. The company representative was notified by the master immediately after addressing the resultant safety concerns between 0230 and 0300 hours. The initial notice of marine casualty to the Coast Guard took place at 1415 hours, nearly 12 hours after resultant safety concerns were addressed by the crew.

The marine employer must have a sufficient number of alcohol testing devices readily accessible on board the vessel to determine the presence of alcohol in the system of each individual who was directly involved in the SMI. Vessel did not have alcohol testing devices onboard and did not obtain the devices within 2 hours.

Each ring life bouy must be marked with type II retro-reflective material; Ring bouy on bridge had worn tape. Waterlight on starboard side was expired. 46CFR185.604(i) & 46CFR180.70(d)(1)

A hinged watertight door must be fitted with a quick action closing device; dog arms were loose in galley watertight door. 46CFR179.330(b)

Painter line was not attached to the weak link and would not deploy the liferaft in an emergency.

Engine room CO2 pilot bottle line had fracture at compression fitting due to vibrations rendering pull stations inoperable.

The vessel shall have strong and effective rudder stops. The port side rudder stop had a crack at the base. Repair at next available drydock (30 days).
