THE PURPOSE OF THIS REGULATION IS TO PREVENT THE IGNITION OF COMBUSTIBLE MATERIALS OR FLAMMABLE LIQUIDS. FOR THIS PRUPOSE THE FOLLOWING FUNCTIONAL REQUIREMENTS SHALL BE MET: IGNITION SOURCES SHALL BE RESTRICTED; PSCO OBSERVED POOLED OIL THROUGHOUT THE ENGINE ROOM DUE TO A PREVIOUSLY RUPTURED GASKET ON THE FUEL HEATER SLUDGE TANK

The fire resistance of doors shall be equivalent to that of the division in which they are fitted... PSCO observed multiple doors in the engine room missing its locking mechanism creating openings. Additionally, multiple doors failed to properly closed. 74 SOLAS 20 11-2/9.4.1.1.5.1

Rescue boats shall be stowed in a state of continuous readiness for launching in not more than 5 min. Crew was unable to start rescue boat upon request.

The master... must maintain a ballast water management plan that has been developed specifically for the vessel. The BWM plan has not been updated to reflect recent changes to BWM certificate. Additionally, BWM plan does not address system limitations when conducting tank stripping operations for inspection. According to C/O treatment system cannot be operated while discharging for stripping operation.

Cylinders must always be secured and, when not in use, they must be stowed in a rack in an upright position, with the valve protection cap in place. PSCO observed 02 oxygen cylinders not secured and 01 cylinder not in use with no valve protection cap in place.

Every steam pipe and every fitting connected thereto through which steam may pass shall be so designed, constructed, and installed as to withstand the maximum working stresses to which it may be subjected. PSCO observed leaks from steam piping in the vicinity of the vessel's fuel pumps, bunker heating line for main engine, electrical store room, above the main engine, boiler, generator purifier room. 74 SOLAS (2004) II-1/33.1

After survey of the ship under regulations 7,8,9, or 10 has been completed, no change shall be made in the structural arrangements, machinery, equipment and other items covered by the survey, without the sanction of the administration. An epoxy repair was found on the port side of the M/E crank case and missing dog on port side crankcase hatch. 74 SOLAS (2020) I/11(b)

15109- The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be established by the company. The vessel's SMS states that releaving person shall inspect the engine room and fill in the checklist "Change of Duty Person." However there was no discrepancies noted on any of the checklists or the engine logbook addressing the material conditions of the engine room. As stated in deficiencies # 2 and # 3 the PSCO observed significant discrepancies throughout the machinery spaces, none of which were documented in the running worklist as indicated in the guidelines for engineers. These also provide evidence of a serious lack of effectiveness and implementation of the vessel's planned maintenance system and required routine inspections/rounds by the ship's crew. Due to objective evidence in the above deficiencies the vessel is not in substantial compliance with relevant conventions. The Captain of the Port questions the adequacy and overall implementation of the vessel's SMS under the ISM Code. An external audit is required to determine whether the ship is operating in accordance with the ISM Code. Provide a copy of the Audit report to the USCG when completed. 74 SOLAS (2020) IX/3.1 ISM Code 10.1

07126-Means shall be provided to control leaks of flammable liquids. PSCO observed excessive oil leaks and fuel accumulations found around the following: fuel pumps, base of the main engine, crank case oil mist detector, main engine fuel injectors, # 1 & # 2 diesel generator fuel injectors, under the steam turbo, and throughout the engine room. This in conjunction with the increased risk of fire associated with exposed electrical wires noted in def #3 poses a serious risk of fire. 74 SOLAS II-2/4.1.2

09209-Terminations and joints in all conductors shall be made as to retain the original electrical mechanical, flame retarding and, where necessary, fire-resisting properties of the cable. PSCO observed dead-end wires & broken electrical sheathing near fuel leaks by the vessel fuel pumps, oil soaked junction boxes on the M/E crank case, near the steam turbo and throughout the engine room. 74 SOLAS (2004)II-1/45.5.6

07113-All pipes to which this regulation refers shall be of steel or other equivalent materials to the satisfaction of the Administration. PSCO observed temporary repairs to the discharge end of the #1 Fire Pump and temporary repairs to the sewage pipes. ICLL (88 Proto) 22.6

All lifesavings appliances shall be in good working order and ready for immediate use. Vessel reported an inoperable rescue boat crane. 74SOLAS(14) CH.3 Reg. 20.2

Ships shall be designed, constructed and maintained in compliance with the structural, mechanical and electrical requirements of a classification society. Service air pipe was found to have temporary repairs made due to a leak. 74 SOLAS (04) II-1/3-1

Means shall be provided to control the leaks of flammable liquids. Fuel oil bunker overflow line is leaking. 74 SOLAS (14) II-2/4.1.1

74SOLAS(2014) II-2/5.1.2 40ac Purpose of this regulation is to limit the fire growth potential in every space. For this purpose the following requirements shall be met: Means of control for flammable liquids in the space shall be provided. PSCO observed oil soaked lagging on generator #3 and M/E.

<2050> 74SOLAS(04)II-1/26.1 (40ac): The machinery, boilers, and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they were intended. Overboard discharge piping is excessively corroded due to lack of maintenance causing leakage during operation.

<0750> 74SOLAS(04)II-2/4.3 (40ac) In spaces where penetration of oil products is possible, the surface of installation shall be impervious to oil or oil vapors. There were multiple leaking flanges resulting in oil-soaked lagging throughout the engine room.

<0950> 74SOLAS(04)II-1/40.1.3 (40ac). Electrical installations shall be such that the safety of the passengers, crew, and ship from electrical hazards will be ensured. Drainage piping above fuel oil purifier motors is excessively corroded due to lack of maintenance, and presents potential electrical hazards.

<0710> 74SOLAS(04)II-2/5.1.2 (40ac). The purpose of this regulation is to limit the fire growth potential in every space of the ship. For this purpose, the following functional requirements shall be met: means of control for flammable liquids in the space shall be provided. Aft-main engine blower was leaking oil excessively and dripping onto the deck.

<0830> 74SOLAS(04)II-1/45.5.5 (40ac). Cables and wiring shall be installed and supported in such a manner as to avoid chafing or other damage. Electrical conduit did not support cable in the overhead causing chafing of electrical cables.

74 SOLAS (09) V/7.1.4 40a.c. EVERY SHIP SHALL BE PROVIDED WITH: A RECEIVER CAPABLE OF RECEIVING INTERNATIONAL NAVTEX SERVICE BROADCASTS IF THE SHIP IS ENGAGED ON VOYAGES IN ANY AREA WHICH AN INTERNATIONAL NAVTEX IS PROVIDED. NAVTEX IS CURRENTLY UNABLE TO RECEIVE MESSAGES.

74 SOLAS (09) II-2/4.1.1 10c. THE PURPOSE OF THIS REGULATION IS TO PREVENT THE IGNITION OF COMBUSTIBLE MATERIALS OR FLAMMABLE LIQUIDS. FOR THIS PURPOSE, THE FOLLOWING FUNTIONAL REQUIRMENTS SHALL BE MET: MEANS SHALL BE PROVIDED TO PREVENT LEAKS OR FLAMMABLE LIQUIDS. LUBE OIL PURIFIER HAD EXCESSIVE OIL IN THE OIL PAN.
