Fuel oil leaks were found throughout the purifier room. The fuel oil leaks have caused fuel oil to accumulate, presenting a fire hazard.

OIL FILTERING EQUIPMENT SHALL BE PROVIDED WITH ALARM ARRANGEMENTS TO INDICATE WHEN THIS LEVEL CANNOT BE MAINTAINED. THE SYSTEM SHALL ALSO BE PROVIDED WITH ARRANGEMENTS TO ENSURE THAT ANY DISCHARGE OF OILY MIXTURES IS AUTOMATICALLY STOPPED WHEN THE OIL CONTENT OF THE EFFLUENT EXCEEDS 15 PPM. IN CONSIDERING THE DESIGN OF SUCH EQUIPMENT AND APPROVALS, THE ADMINISTRATION SHALL HAVE REGARD TO THE SPECIFICATION RECOMMENDED BY THE ORGANIZATION. TO AVOID WILFUL MANIPULATION OF THE 15 PPM BILGE ALARMS, THE 15 PPM BILGE ALARM SHOULD BE SO CONSTRUCTED THAT THE ALARM IS ALWAYS ACTIVATED WHENEVER CLEAN WATER IS USED FOR CLEANING OR ZEROING PURPOSES. WHILE CONDUCTING THE SIMULATION TEST FOR THE 107 OIL CONTENT METER, THE OPERATIONAL TEST OF THE 15 PPM ALARM, AND FUNCTION OF THE 3 WAY OVERBOARD VALVE, PSCO IDENTIFIED THE UNRELIABILITY OF THE OIL CONENT METER (OCM), FRESH WATER FLUSH VALVE/CONTROL, AND 3 WAY OVERBOARD VALVE. WITH THE OCM IN A "OK" STATUS, THE 2ND ENGINEER SHIFTED THE FRESH WATER FLUSH VALVE TO FLUSH. THE OCM STAYED IN A "OK" STATUS AND THE OCM DISPLAY FROZE WITH 5 PPM ON THE SCREEN AND NO ALARM WAS RECEIVED ON THE OCM. THE OVERBOARD VALVE DID NOT CHANGE STATE AND REMAINED IN THE CLOSED POSITION. THE 2ND ENGINEER THEN SHIFTED THE FRESH WATER FLUSH VALVE TO NORMAL OPERATION AND THE OVERBOARD VALVE REMAINED IN THE CLOSED POSITION WITH A "OK" STATUS AND NO ALARMS ON THE OCM.
