Every launching appliance shall be fitted with brakes capable of stopping the descent of the survival craft or rescue boat and holding in securely when loaded with its full complement of persons and equipment. The winch brake cable on the starboard lifeboat was found frayed and unable to be attached to the handle for operation from inside the lifeboat. 74 SOLAS (24) III/16.1, LSA Code Ch. VII/6.1.2.11.; MISLE Code 11129; Action Code 10c.

Where "A" class divisions are penetrated, such penetrations shall be tested in accordance with the Fire Test Procedures Code, subject to the provisions of paragraph 4.1.1.5. Such penetrations shall be suitably insulated by extension of the insulation at the same level of the division. Psco found holes in fire boundaries on bulkheads throughout the main deck.

During vessel's transit into the Port of Hueneme, vessel experienced a failure in the no.1 cylinder fuel pump. C/E explained due the low lubrication while using LSGO the rack on the fuel pump requires regular lubrication. Class to attest to m/e proper operation and vsl's compliance with SMS while operating in NAECA using LSGO. SOLAS (14cons) Ch. II-I reg. 26.1. (17a/c)
