A vessel on which a pre-engineered fixed gas fire extinguishing system is installed must have the following equipment at the operating station, an audible alarm that sounds upon discharge. The pre-engineered fixed gas fire extinguishing system required audible alarm that sounds upon discharge was OOC. Repair the audible alarm and prove operation in the presence of the attending marine inspector.

A submersible electric bilge pump may be used as a power bilge pump required by Table 182.520(a) only on a vessel of not more than 65 feet in length carrying not more than 49 passengers, other than a ferry. The submersible electric bilge pump for the forward space motor was burned out rendering the pump inoperable. Replace with a submersible electric bilge pump that is UL approved and meets the requirements as described by 46 CFR 182.520 and test in the presence of the attending marine inspector.

A vessel not required to have a power-driven fire pump by § 181.300 must have at least three 2 ½ gallon buckets. The three fire buckets were 2 gallons. Provide the required three 2 ½ gallon buckets to the satisfaction of the attending marine inspector.

A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radiobeacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. 2. Vessel Safety Radiotelephony Certificate was expired. Provide valid Safety Radiotelephony Certificate to the satisfaction of the attending marine inspector.

The owner, managing operator, or a qualified servicing facility as applicable must conduct the following inspections and tests: (1) Portable and semi-portable extinguishers must be inspected and maintained in accordance with NFPA 10 as amended here: Monthly inspections required by NFPA 10 may be conducted by the owner, operator, person-in-charge, or a designated member of the crew. Monthly inspections required by NFPA 10 may be conducted by the owner, operator, person-in-charge, or a designated member of the crew were not conducted. Where monthly manual inspections are conducted, provide records on a tag attached to the fire extinguisher, on an inspection checklist maintained on file, or by an electronic method to the satisfaction of the attending marine inspector.

The owner, managing operator, or a qualified servicing facility as applicable must conduct the following inspections and test portable and semi-portable extinguishers must be inspected and maintained in accordance with NFPA 10. Annual maintenance has not been conducted since 2023. Conduct applicable maintenance to the satisfaction of marine inspector. 46 CFR 176.810(b)(1)

All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules. Starboard running light found inoperable. Light needs to be repaired and comply with Navigation rules to the satisfaction of the attending marine inspector. 46 CFR 183.420

Fuses must be of the cartridge type only and be listed by Underwriters Laboratories or another independent laboratory recognized by the Commandant. Two ANL 50 type fuses in the machinery space starboard side are not the approved type. Replace fuses according to this regulation to the satisfaction of the attending marine inspector. 46 CFR 183.830(1)

Each connection to a conductor or terminal part of a conductor must be made within an enclosure and have either: (1) A pressure type connector on each conductor;(2) A solder lug on each conductor; (3) A splice made with a pressure type connector to a flexible lead or conductor; or (4) A splice that is soldered, brazed, or welded to a flexible lead or conductor. Two Quick splice terminals designed for general automotive were used behind the main panel to connect the radio. These are not approved and must be removed and reconnected according to the above requirement and to the satisfaction of the attending marine inspector. 46 CFR 183.340 (g)

Electrical equipment on a vessel must be installed and maintained to: (a) Provide services necessary for safety under normal and emergency conditions. Nine of the main electrical panel breaker handles were broken off. Replace all damaged electrical breakers to that meet 46 CFR 183.380 (m) to the satisfaction of the attending marine inspector. 46 CFR 183.200

At least one ring life buoy must be fitted with a lifeline… of at least 18.3 meters (60 feet) in length. The lifeline on the vessel's ring buoy was 50 feet in length. Replace the lifeline to the satisfaction of the attending marine inspector. 46 CFR 180.70(c)(2)

Each circuit breaker must meet UL 489 or other standard specified by the Commandant, and be of the manually reset type designed for: (1) Inverse time delay; (2) Instantaneous short circuit protection; and (3) Switching duty if the breaker is used as a switch. The Blue seas 285 series breakers installed in the machinery room starboard side are not approved. Replace breakers with approved type according to the above regulation to the satisfaction of the attending marine inspector. 46 CFR 183.380 (m)

Electrical equipment on a vessel must be installed and maintained to: (b) Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. The vessel has an exposed electrical conductor in the steering space. (1) Remove exposed electrical wiring or, (2) Encapsulate exposed wiring to the satisfaction of the attending marine inspector. 46 CFR 183.200

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. Medical kit found expired. Replace to satisfaction of the attending marine inspector. 46 CFR 184.710

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of the hull structure and its appurtenances, including the following: (4) Inspection of all railings and bulwarks and their attachment to the hull structure. The rear port quarter top railing had a hole worn from service. Repair the hole to the satisfaction of the attending marine inspector. 46 CFR 176.802 (a)

A vessel must comply with the applicable requirements for any radio... including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. No crew members found to have a marine radio operator permit. Obtain permit(s) to the satisfaction of the attending marine inspector. 46 CFR 184.502

A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radio-beacon (EPIRB) installations… including installation certificates issued by the FCC, as set forth in 47 CFR part 80. EPIRB did not have registration sticker. Replace EPIRB sticker to the satisfaction of the attending marine inspector. 46 CFR 184.502

The master and crew of a vessel will be familiar with the content of and have mounted at the operating station, emergency instructions containing the actions to be taken in the event of fire, heavy weather, or man overboard conditions. Emergency instructions in the event of fire, heavy weather, or man overboard conditions were not posted onboard. Identify and post instruction to the satisfaction of the attending marine inspector. 46 CFR 185.510 (a)

A vessel must be provided with bilge pumps in accordance with Table 182.520(a). (1) The pump is listed by an independent laboratory as meeting the requirements in UL 1113 (incorporated by reference; see 46 CFR 175.600). Bilge pumps in the compartment forward of the machinery space and in the steering space are not the correct UL approved type. Replace these two bilge pumps with UL 1113 approved bilge pumps and test the operation in the presence of the attending marine inspector. 46 CFR 182.520

Each EAP education program must include at least the following elements: display and distribution of informational material; display and distribution of a community service hot-line telephone number for crewmember assistance, and display and distribution of the employer's policy regarding drug and alcohol use in the workplace. The required information and community service hot-line telephone numbers were not on board. Provide the above employee assistance information to the satisfaction of the attending marine inspector. 46 CFR 16.401 (a)

An EAP training program must be conducted for the employer's crewmembers and supervisory personnel. The training program must include at least the following elements: the effects and consequences of drug and alcohol use on personal health, safety, and work environment; the manifestations and behavioral cues that may indicate drug and alcohol use and abuse; and documentation of training given to crewmembers and the employer's supervisory personnel. Supervisory personnel must receive at least 60 minutes of training. No record of required crew and supervisor training. Conduct required training as described above and provide documentation of the training to the satisfaction of the attending marine inspector. 46 CFR 16.401 (b)

On passenger decks of a ferry or of a vessel on an excursion trip the following must be installed: Bars, slats, rail courses, or an equivalent spaced at intervals of not more than 100 millimeters (4 inches). 46CFR177.900(g)(1)(iii)

The owner or managing operator or a qualified servicing facility shall conduct inspections and tests as applicable for fixed gas fire extinguishing systems and must provide evidence of the required servicing to the marine inspector. Vessel's fixed gas fire extinguishing system has not been inspected or tested since May 2021. Provide satisfactory evidence of required inspections, maintenance procedures, and tests by a qualified servicing facility. 46 CFR 176.810(b)

A valid certificate of number for that vessel issued by the issuing authority in the State in which the vessel is principally operated is required. A valid state registration was not on board. Provided evidence of state registration. 33 CFR 176.21(1)

Portable watertight containers for distress flares and smoke signals shall be of a bright color, and containers shall be clearly marked in legible contrasting letters at least 12.7 millimeters (0.5 inches) high: “DISTRESS SIGNALS”. Distress signals not stowed properly. 46 CFR 185.614

Employers must ensure that they and their crewmembers meet the requirements of this part. Designated Employee Representative(DER)training not provided. Alcohol strips expired. Hot line numbers not made available to crew. 46 CFR 16.203(a)

The EPIRB test required by paragraph (a) shall be logged or otherwise documented, as applicable. EPIRB test not logged properly. 46 CFR 185.728(c)

Abandon ship and man overboard drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. The drill entry shall include the following information: Date of the drill and training; and General description of the drill scenario and training topics. 46 CFR 185.520(f)

A vessel on an excursion trip must have bars, slats, rail courses, or an equivalent spaced at intervals of not more than 100 millimeters (4 inches). Vessel has removed courses along the starboard side railing. Vessel must replace missing courses along starboard side railing prior to carrying children and within 30 days to the satisfaction of the attending marine inspector. Failure to replace courses within 30 days will result in suspension of Certificate of Inspection.

Keeper nut for steering connection to the rudder was fractured. A vessel must be provided with a main steering gear that is of adequate strength and capable of steering the vessel at all service speeds.

Auxiliary steering tiller is too long and blocked by engine cover. An auxiliary means of steering that is Controlled from a location that permits safe maneuvering of the vessel and does not expose the person operating the auxiliary means of steering to personnel hazards during normal or heavy weather operation.

Provide cover for stbd cabin light. Operator shall be prepared to conduct tests and have the vessel ready for inspection of electrical equipment and systems

Fireboy fixed firefighting shut down system does not work as designed. Engine restarts without override switch being engaged.

The vessel's master did not have their radio operator permit present on board the vessel available for inspection as required by 47 CFR 80.175; Provide Marine Radio Operator permit.

To include position information in a distress alert, the VHF-DSC radio should be properly connected to a GPS Receiver or have a VHF-DSC marine radio with integral GPS. Vessel's VHF-DSC RADIO is not properly connected to a GPS received.

Pitting around elbows of weld of platform beams. Replace 3, 4, and 5 suuport beams attached to swim platform.

Cracking evident in fiberglass around rudder log. Repair delamination around rudder post, specifically starboard side aft of rudder post.
