An immersion suit, complying with the requirements of section 2.3 of the Code or an anti-exposure suit complying with section 2.4 of the Code, of an appropriate size, shall be provided for every person assigned to crew the rescue boat or assigned to the marine evacuation system party. PSCO observed three immersion suits with torn seams in locations of the entry zipper and boot seams. While onboard the vessel received a requisition containing two replacement immersion suits.

Internal combustion engines of a cylinder diameter of 200 mm or a crankcase volume of 0.6 m3 and above shall be provided with crankcase explosion relief valves of a suitable type with sufficient relief area. PSCO observed one explosion relief valve wired shut and other explosion relief valves that appeared to be over tightened causing deformation of the internal structure. Resolved 31 July 2017
Resolution: Received, reviewed and accepted class report from ClassNK stating the deficiency was rectified. Fuel injection nozzles, plunger and barrel for all three generators were renewed. LSMGO heating temperature was too high and adjusted to 45 degree C.
