Boilers, main and auxiliary machinery, and piping systems shall conform to the requirements of subchapter F (Marine Engineering) of this chapter, except as otherwise provided for in this subchapter. The last 1.0 meter (3.3 feet) of vapor piping upstream of the vessel vapor connection and each end of a vapor hose must be— (1) Painted in the sequence of red/yellow/red. The width of the red bands must be 0.1 meter (0.33 foot) and the width of the middle yellow band must be 0.8 meter (2.64 feet); and (2) Labeled with the word “VAPOR” painted in black letters at least 50.8 millimeters (2 inches) high. Port side vaporline is not painted with required information /colors. 46 CFR 39.2001(h)

All tank vessels, the construction or conversion of which is started on or after July 1, 1951, shall conform to the requirements in this subpart in construction of hulls. The hull and deckhouses shall be of steel or iron construction except that the pilothouse and decks over quarters may be constructed of wood. Scantlings, material, and workmanship, subdivision of cargo spaces, fitting of cofferdams, and testing of tanks shall be at least equivalent to the requirements of the American Bureau of Shipping or other recognized classification society. The following voids hatch coamings have excessive wastage on knife edge. #1P,#2P,#4P,#5P,#1S,#2S,#3S,#4S, port forward rake hatch and aft Starboard rake hatch. Crop and renew hatch coamings at next scheduled dry dock to the satisfaction of the attending marine inspector.

A pressure gage shall be installed for each pump discharge, and it shall be located at a point visible with respect to the pump controls. Pressure gauges for cargo pump where not visible from inside the operating station.

All transfer piping and hoses must be hydrostatically tested at one and a half (1 1/2) times its maximum allowable working pressure. The transfer piping on the barge was tested at its working pressure (150 psi). Tank barge is to limit its transfer pressure to no more than 100 psi until a new hydrostatic test at 225 psi is conducted.

The ring, lines, and letters of the load line shall be permanently marked on the sides of the ship to the satisfaction of the Administration. The load line markings on the tank barge did not seem to be permanently marked and were painted with an aft trim (not parallel to the freeboard deck).

The annual inspection shall include an inspection of the structure is adequate and satisfactory for the route and service intended. It was noted that the starboard side aft stairs leading into the raised walkway aft of the machinery space are wasted and broken.

All portable extinguishers on board tank barges shall be of an approved type. There are 2 portable fire extinguishers which brackets are broken. The fire extinguisher by the entrance to the compressor space is being held by thin ropes. There is another extinguisher by the starboard side access ladder to the engine compartment that is placed inside its box but no cover is present. The extinguisher could fall at any time.

Containment systems on board tank barges with a capacity of 250 barrels or more carrying oil as cargo in bulk must have mechanical means of closing each drain. It was noted that there is a overall practice of not installing the mechanical plugs on containment systems and on scuppers.

Each switchboard must meet the location and installation requirements in section 8.2 of IEEE or IEC 60092-302. The switchboard’s lights were not operating.

Exhaust pipe installations must conform to the requirements of ABYC P-1. The #1 generator, the port and starboard fuel oil pump’s prime movers’ exhaust piping is showing signs of leaks as evidenced by the soot accumulated on the lagging.

A pressure gauge shall be installed for each pump discharge. The pressure gauges located at the outer bulkhead by the cargo pumps for the starboard gas oil and port fuel oil cargo piping are broken.

Annual inspection. Your vessel must undergo an annual inspection within 3 months before or after each anniversary date. Barge miss annual inspection date.

A barge must undergo an annual examination every year. A barge must undergo a MARPOL ANNEX I examination every year. Inspections deferred for 90 days due to lockdown measures implemented in Puerto Rico.

Stern Box (STBD): Crop and renew side shell plating and damaged structural members between frames 44 and 43 and overhead and side long 3.

Stern Box (PORT): Crop and renew side shell plating and damaged structural members (side longs, tripping brackets) between frames 44 and 43 and overhead and side long 3.

Stern Box (PORT, inboard): Crop and renew fractured frame and tripping bracket between frame 44 and 43.

Void Space #3 (PORT): Repair broken welds on side long #2 between frames 26-25.

Void Space #1 (PORT): Replace section of side long #2 with same size side long.

Repair all damaged void space hatch covers and hinges.

Each nonmetalic transfer hose shall be tested to at least 1.5 times the MAWP (225 psi). Vessel had 09 hoses tested to the MAWP (187 psi).

Each transfer pipe must not leak under static liquid pressure of at least 1.5 times the MAWP (225 psi). Vessel's GAS OIL pipe was test to the MAWP (187 psi).

Portside GAS OIL pump Emergency Stop had a temporary spliced extension cord tied into the system.

#35 transverse frame in the #5 starboard void was warped about 60% in the upper part of the frame.

Portside #3 void, frame #20, 3rd from the bottom side long has a fractured weld.

Portside #2 void, in between frames 12 and 13, 4rd from the top side long has a fractured weld.

Remove or properly service EPIRB onboard tank barge.

Remove or properly service inflatable life raft onboard the tank barge.

Prior to cargo ops, provide proof of hydrostatic testing of cargo hoses, proper marking of cargo hoses, and written records of cargo hose testing.

Prove testing of cargo pump pressure gages.

Prove (3) fire extinguishers in machinery space areCoast Guard approved.

Prove service generators located in auxiliary machinery room have been derated.

Prove battery chargers meet enclosure requirements.

Prove cables for battery chargers meet both flexible and shipboard cable requirements.

Prove proper operation of ground detection.

Prove OPA 90 spill kit meets requirements for 7 barrels.

Prove proper operation of generator overspeed trips.

Provide PIC designation list.

It was discovered that vessel's ISE dates had expired on 30 July 2012. Inpectors conducted an ISE on 29 August 2012, vessel was operating for 30 days with an invalid COI.

33 CFR 155.1040(k) The barge-specific appendix must be amended to include the capacities of all ballast tanks onboard this barge or state that these tanks are not applicable.
