Before the ship leaves port and at all times during the voyage, all lifesaving appliances shall be in working order and ready for immediate use. PSCO observed broken throttle handle on the rescue boat. In addition, the rescue boat manual did not indicate a a throttle was installed to engage the engine into forward and reverse.

The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be established by the company. The company should ensure that inspections are held at appropriate intervals; any non-conformity is reported with its possible cause; appropriate corrective action is taken, and records of these activities are maintained. As referred in deficiency 01 the rescue boat throttle is broken diagrams referenced did not show a throttle was installed in the design standards. Inspections as per the company's SMS inspection form VS.OPS.047 indicated satisfactory inspection results. PSCO observed last usage of incinerator was conducted on 06AUG2020, per their Garbage record book. Observed inspection records conducted on 17AUG2020 showing all satisfactory even though three was a plastic bag over the flame detector, bypassing a safety system as referenced in deficiency 03. An internal ISM audit and record or corrective action is required within three months.

At 0330 est during the inbound transit to Garden City Container Terminal, Savannah, GA, the ships #2 main air compressor piston crown broke causing the compressor to fail. During this transitthe engine was ordered dead slow ahead/astern multiple times depleting the start air reciever below the "low start air inlet" setpoint of 15kg/cm2 causing a subsequent 5 low start air pressure alarms-see formB
