Machinery and associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended. While vessel was transiting outbound Chief Engineer discovered an indicator valve on the main engine cylinder head #10 was blowing down strong exhaust gases resulting in an abnormal noise. Indicator valve had a failed block screw. 74SOLAS(04) II-1 / 26.1

In a ship in which oil fuel is used, the arrangements for storage, distribution and utilization of the oil fuel shall be such as to ensure the safety of the ship and persons on board. Five gallons of fuel oil and multiple oil soaked rags observed in containment of purifier space due to a 10cm crack in very low sulfur service tank leaking approximately 60 liters/day. Additionally, main engine has significant accumulation of fuel oil around all cylinder heads and over the entire body of the engine presenting insufficient cleanliness of engine room and multiple fire hazards throughout the engine room.
