PSCO's observed multiple containers with residue of flammable fuel oil stacked in the void spaces adjacent to generators and multiple elctrical batteries stored behind the switchboard in the emergency generator room.

The company should ensure that all personnel involved in the company's safety management system have an adequate understanding of relevent rules, regulations, codes and guidelines. PSCO observed that the crew did not properly fill out vessel's hot work permit or obtain a signed standard letter by welding company prior to hot work commencing as per SMS. Request an internal audit be conducted within 90 days. SOLAS 74 (a), 2020 cons., IX/3, ISM Code 6.4, Code a, b, c.
