A mechanical exhaust system must consist of at least two ventilation exhaust ducts and the exhaust blowers must be interlocked with the starting and ignition switch. A red warning sign at the switch must state that "The blowers must be operated for 3 minutes prior to starting the engines". The starboard blower was inop, the blowers were not interlocked with the starting switch and the red warning sign was missing.

Fuel shutoff valves must be installed as close to the tank as possible and if inside the machinery space, it must be located so that the operator does not have to reach more than 12 inches into the machinery space. The valve was not reachable from outside the space.

A seat must be constructed to minimize the possibility of injury. Both forward deck seats have brackets broken and/or missing.

On a Lakes, Bays, and Sounds route, a vessel must carry 03 hand red flare distress signals and 03 orange smoke distress signals. 03 hand red flares can be used as a substitute for the 03 orange smoke distress signals. Vessel had only 03 hand red flare distress signals. 03 orange smoke distress signals or 03 additional hand red flare distress signals are required.

Small passenger vessels must have a valid Radio Safety Telephony Certificate (FCC form 824). Obtain valid certificate.

First aid kit must meet standard of ISO 18813(2006) or meet equivalent standards. Onboard first aid did not meet standards. Replace kit with on that meets prescribed standards.

At each inspection for certification, the marine inspector may require that a man overboard drill be held under simulated emergency conditions specified by the inspector. Because the vessel has a COI manning scale allowing vessel to operate with only a Master for operations lasting no greater than twelve (12) hours, a man overboard drill was conducted where the master was supposed to recover a test dummy unassisted. The master was unable to recover the test dummy unassisted*. The vessel is prohibited from operating with only a Master, and must operate with a crew of 01 Master and 01 Deckhand until a successful man overboard drill is witnessed by the attending USCG marine inspector under the operating conditions of "Master Only" (i.e. the master must make recovery unassisted). *Master has not yet received training to recover man-overboard without assistance.

Each vessel must have a stability letter issued by the Coast Guard that sets forth conditions of operation. Current stability letter, issued 29 July 2022, states that no weights shall be added, removed, altered and/or relocated without authorization of cognizant [name withheld] installed vessel engines result in weight changes to the vessel. Either a new stability proof test must be conducted and new stability letter is issued by cognizant OCMI, or owner must prove that the new engines are equal in weight to the previous engines. Vessel is restricted to 6 or fewer passengers.

Fixed-gas fire extinguishing systems must be inspected and tested as per 46 CFR 176.810(b). Complete annual servicing for the fixed fire fighting system.

The construction and arrangement of a vessel must give consideration to provisions of a seaworthy hull. Recent maintenance caused open wire/piping penetration holes to remain open between outboard well deck and aft engineering space. Close or plug unused openings.

A vessel must carry either a first aid kit approved under 160.041 or an equivalent. Replace expired first aid kit.

Each vessel must have a stability letter issued by the Coast Guard that sets forth conditions of operation. Current stability letter, issued 16 October 2017, states that no weights shall be added, removed, altered and/or relocated without authorization of cognizant [name withheld] installed vessel engines result in weight changes to the vessel. Vessel is restricted to carriage of 6 or fewer passengers for hire until new stability proof test is conducted and new stability letter is issued by cognizant OCMI.

There must be a means of accurately determining the amount of fuel in each fuel tank. Repair port side fuel gage.

An adult life jacket must be provided for each person carried onboard. 02 life jackets were found in disrepair. Replace the faulty life jackets. Maximum passenger capacity is reduced to 20 persons (adult) until the faulty life jackets are replaced.

Double hose clamping of flexible hoses is required where practicable. Double clamp fuel suction line from starboard fuel tank.

Electrical equipment must be installed and maintained to protect the vessel from electrical hazards. Repair fuse for the forward bilge pump at console switch.

FCC Station License expired

Dead-end wires found underneath operator's station. Cap off and secure or remove dead-end wires.

First Aid kit had expired medicines. Replace expired medicines/First Aid kit with new.

Charts, Coast Pilot [name withheld] and Tide Table publications were out of date. Update charts and nautical publications with latest versions.

Repair or replace starboard exhaust blower for gasoline tank space.

Electric bilge pumps must be listed as meeting UL 1113.

Portable fire extinguisher at the operating station must have a minimum required rating of UL 10-B:C.

Tighten bonding strap on starboard fuel tank.

Non-metallic hose on starboard fuel tank must meet J1942 standard. Replace or provide proof of certification.

2 PFDs must be marked with the vessel name.

Shaun Simoneaux and [name withheld] shall not operate in a 'master only' condition until satisfactorily conducting a Man-Overboard Drill in a reduced crew ('Master-Only') condition in the presence of the attending USCG Marine Inspector.

Prove proper operation of audible bilge alarm.
