AT EACH LOCATION THERE SHALL BE CLEAR INSTRUCTIONS RELATING TO THE OPERATION OF THE SYSTEM HAVING REGARD TO THE SAFETY OF PERSONNEL. PSCO OBSERVED A LACK OF WRITTEN INSTRUCTION AT FIRE CONTROL STATION. INSTRUCTIONS WERE PSOTED PRIOR TO END OF PSC EXAM. 74SOLAS(20) II-2/10.4.1.1.1. FSS CODE 5.2.1.3.3.

Following installation, the master, owner, operator, agent, or person in charge of the vessel subject to this subpart must properly maintain the BWMS in accordance with all manufacturer specifications. Ballast water management system was reported inoperable. Vessel shall not conduct ballast water exchange within the waters of the United States. 33 CFR 151.2025

The machinery, boilers and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons onboard, due regard being paid to moving parts, hot surfaces and other hazards. PSCO observed steady hydraulic leak on port side steering gear.

Resolution: Recieved, reviewed and accepted class report from RINA stating the deficiency was rectified. Broken seal of the port side ram had been replaced with a spare seal.
