All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from fire or machinery failure. MI discovered a fuel leak on the generator fuel filter. 46 CFR 182.100

Shutoff valves, installed so as to close against the fuel flow, must be fitted in the fuel supply lines...The shutoff valve at the tank must be manually operable from outside the compartment in which the valve is located, preferably from an accessible position on the weather deck. If the handle to the shutoff valve at the tank is located inside the machinery space, it must be located so that the operator does not have to reach more than 12 inches into the machinery space and the valve handle must be shielded from flames by the same material the hull is constructed of, or some noncombustible material. Generator fuel shutoff valve located at the fuel tank connection is not fitted with a remote shutoff. Prior to using the ship's generator, install remote pull-station outside the machinery space and test in the presence of a Coast Guard Marine Inspector.

A submersible electric bilge pump may be used as a power bilge pump only on a vessel of not more than 65 feet in length, carrying not more than 49 passengers provided that a positive shutoff valve is installed at the hull penetration. Forward bilge pump and forward bilge pump in the engine room do not have positive shut off valves at the skin of the vessel. Install prior to next operating season. 46 CFR 182.520(e)(7)

A vessel must carry either a first aid kit approved under 160.041 or a kit with equivalent contents and instructions. Ammonia inhalants were expired and only 4 tablets of aspirin were provided. Outfit first aid kit in accordance with 46 CFR 160.041-4(b).

Each ship transporting more than six passengers for hire equipped with a radiotelephone station in accordance with Part III of Title III of the Communications Act must carry a radio operator... Master did not have a marine radio operator permit or other appropriate license. Provide proof of license. 47 CFR 80.159(e)

Vessel's starboard MDE suffered a catastrophic failure. Prove proper operation of vessel's propulsion prior to carrying passengers.

EPIRB testing and lifesaving maintenance is not being logged. Logs pertaining to drills are inconsistent and not inclusive of all the requirements set forth in 46 CFR 185.520. The Coast Guard will issue a medical certificate to a mariner meeting the medical and physical standards for merchant mariners. Master of the vessel (Taylor) does not have a copy of her current medical certificate. Provide proof of obtaining a medical certificate within 30 days.
