Fwd Stbd Hull Plating iwo keel, wasted Crop & Renew, 3/8 inch plate, 140 inches by 30 inches. 3. Stbd Hull Plating iwo keel, wasted Crop & Renew, 3/8 inch plate, 46 inches by 20 inches. 4. Aft stack deck wasted & holed, crop & renew, 1/4 inch, 12inches by 18 inches. 5. Stack Deck iwo engine room exterior hatch wasted with multiple holes. Stack deck to be removed and renewed. 6. 2nd/bosen hole, stbd side overhead two holes iwo of deck bit. Crop & renew, with repairs to good material. 7. 2nd/bosen compartment, centerline, all lower/bottoms of transvers shell frames (holed & wasted. Crop & Renewal, to good material. 8. Engine room, lower bottom transvers frame iwo shaft packing gland damaged. Crop and renew frame. 9. Wheel House, portside window holed. Renew window. 10. Two Sea Valves. Not open, removed valve bonnets and open for inspection. 11. Fwd & Aft engine room wtd gaskets, damaged and unable to property seal. Renew gaskets. 12. 3/8 inch hull plating two areas of localized pitting, max depth 1/8 inch. Clad weld or make proper repairs. 13. Port & Stbd Flanking Rudder bearings appear worn, with excessive movement. Provide rudder bearing clearance readings, for consideration. As of 30JUL2025 Company failed to complete Dry-dock and internal structural repairs. Activity closed.

Vessel did not complete Dry Dock inspection by 31 AUG 2024. Complete Dry Dock prior to engaging in towing operations.

Except as provided in paragraphs (d)(2) through (4) of this section, each towing vessel must carry the survival craft specified in Table 141.305 of this section, as appropriate for the towing vessel, in an aggregate capacity to accommodate the total number of individuals onboard. Vessel did not have IBA on board. 46 CFR 141.305 (d) (1)

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. Batteries were dead and hooked to a charging unit. 46 CFR 143.205 (a)

If a dedicated storage cabinet is provided it must be secured to the vessel so that it does not move and must be either: (1) A flammable liquid storage cabinet that satisfies UL 1275 (incorporated by reference, see § 136.112 of this subchapter); or (2) A flammable liquid storage cabinet that satisfies FM Approvals Standard 6050 (incorporated by reference, see § 136.112 of this subchapter); or (3) Another suitable steel container that provides an equivalent level of protection. Fuel canisters were stored in a plastic container not secured to the vessel. 46 CFR 142.225 (c) (1)-(3)

The frame or support of any semi-portable fire extinguisher fitted with wheels must be welded or otherwise permanently attached to a steel bulkhead or deck to prevent it from rolling under heavy sea conditions. Semi-portable extinguisher was ratchet strapped to pilot house bulkhead. 46 CFR 142.230 (c)

The master must ensure that all hatches, doors, and other openings designed to be watertight or weather-tight function properly. Engine room watertight door gasket material was painted over. 46 CFR 140.610 (b)

Sufficient lighting suitable for the marine environment must be provided within crew working and living areas. Engine room terminal block not covered and engine room lighting found hanging by electrical wires. 46 CFR 143.410 (a)

An exposed hazard such as gears and rotating machinery, must be protected by a cover, guard or rail. This is not meant to restrict access to towing equipment such as winches, drums, towing gear or steering compartment equipment necessary for the operation of the vessel. Main engine rotating machinery did not have guard in place. 46 CFR 144.820

Each exhaust pipe from an internal combustion engine which is within reach of personnel must be insulated or otherwise guarded to prevent burns. On a new vessel, each pipe that contains vapor, gas, or liquid that has a temperature exceeding 150 °F (65.5 °C) which is within reach of personnel must be insulated where necessary or otherwise guarded to prevent injury. Turbo Charger lagging was not secured. 46 CFR 144.830

The following systems must be equipped with gauges at the machinery location: (1) Main engine lubricating oil pressure and main engine RPM; (3) Auxiliary generator engine lubricating oil pressure and auxiliary generator engine RPM; (4) Auxiliary generator engine cooling water temperature. Engines did not have local guages as required. 46 CFR 143.230 (c)(1)(3)(4)

To stop the flow of fuel in the event of a fire or break in the fuel line, a remote fuel shutoff valve must be fitted on any fuel line that supplies fuel directly to a propulsion engine or generator prime mover. Pin broke off at shut off valve location. 46 CFR 143.260 (b)

Each towing vessel must be equipped with an industrial type first aid cabinet or kit, appropriate to the size of the crew and operating conditions. Each towing vessel operating on oceans, coastwise, or Great Lakes routes must have a means to take blood pressure readings, splint broken bones, and apply large bandages for serious wounds. Found numerous items in first aid kit expired. 46 CFR 140.435
