The company should establish procedures to ensure the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be established by the company. The objective nature of the deficiencies 02, 03, and 04 demonstrate the vessel is not being maintained to international standards, bringing into question the validity of the safety management certificate issued pursuant to the ISM code. An external audit is required to be submitted prior to departure from port. 74 SOLAS (20) IX/3.1 ISM A/10.1

PSCO found Vessel's installed means of battery charging inoperable. [name withheld] stated to PSCO that Flag Administration and Class Society were not notified of charger failure nor of temporary repairs as required per Safety Management System. 74 SOLAS (20)IV/13.1

Escape routes shall be maintained in a safe condition, clear of obstacles. Two emergency escape doors on "B" deck were unable to be opened in direction of escape route from accommodation space to the exterior of the vessel. PSCO discovered both door handles were missing on the interior side and one came off while attempting to open it from the outside. 74 SOLAS (09) II-2/13.1.2

While conducting cargo deck walk with [name withheld] the PSCO discovered the middle course between the stanchions on port bow edge broken along with wasted supports for steel grating near the starboard anchor windlass. 1999- Other(MARPOL ANNEX II)- Cargo samples in stowage shall be cell-divided in order to avoid shifting of the bottles at sea. PSCO's observed dozens of cargo samples piled on top of other sample bottles. They werent stored securely and were not cell-divided. 74SOLAS(14) VII/10.1 REF. IBC CODE 16.5.2.1
