Where the main propulsion and associated machinery, including sources of main electrical supply, are provided with various degrees of automatic or remote control and are under continuous manual supervision from a control room, the arrangements and controls shall be designed, equipped, and installed that the machinery operation will be safe and effective as if it were under direct supervision. Particular consideration shall be given to protect such spaces against fire. PSCO observed a spacer device placed above the float switch in the high pressure fuel-oil system drain tank for both ship's service generators, preventing the full travel of the installed float and rendering the float-switch inoperable. 74 SOLAS (04) II-1/31.3 10c

All hatchways in position 1 and 2 shall be fitted with hatch covers of steel or other equivalent material. Except as provided in regulation 14(2), such covers shall be weathertight and fitted with gaskets and clamping devices. The arrangements shall ensure that the tightness can be maintained in any sea condition. PSCO observed 50% of cargo hatch clamping devices were unable to be actuated to secure the cargo hatches.
