Electrical installations shall be such that the safety of crew and ship from electrical hazards will be ensured. PSCO observed multiple corroded and damaged exterior light fixtures. Additionally, PSCO observed 01 exterior light casing was cracked. Lighting fixture on stbd, aft fantail was corroded causing the light to hang by only one side of the fixture and its electrical cables. 74 SOLAS (04) II-1/40.1.3

Resolution: DNV GL determined source of leak to be from required fuel change over upon entrance to US ECO zone. All oil wiped from surfaces and crew made tight any pipe connections on the igh pressure fuel oil system.

After a survey has been completed, no change shall be made in the machinery, equipment, and any other items covered by the survey. Upon engine room walk through, PSCO was advised all equipment operational. During walk through, #1 gen actively leaking substantial amount of jacket cooling water and #4 gen currently out of service due to seizure of lube oil pump. Still under investigation with the ship's crew and no notification to flag or class have been made.

Resolution: DNV GL finds that all necessary repairs have been made to AUX engine #1 to arrest any cooling water leaks. DNV GL issued a condition of class to the vessel for AUX engine #4 allowing it to sail till 1/30/2020.

Enclosed space entry and rescue drills should be planned and conducted in a safe manner. Vessel has only one litter, which is rigid and cannot fit into all enclosed spaces.
