A vessel must must be provided with a satisfactory arrangement for draining any watertight compartment. Forward electric submersible bilge pump found non - operational during inspection. Provide operational forward bilge pump. 46 CFR 182.500

A vessel must comply with the applicable requirements for any radio installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. Provide copy of FCC Safety Radio certificate. 46 CFR 184.502

A pre-engineered fixed gas fire extinguishing system must automatically shut down all power ventilation systems and all engines that draw intake air from within the protected space. Fireboy system does not shut down blower in engine compartment. Disconnect power from blower until deficiency rectified. Rectify within 30 days. 46 CFR 181.420(a)(3)

Machinery and equipment installed on each vessel must be suitable for the vessel and its operation and for the purpose intended. Generator prime mover has issue that will not allow generator to provide electrical current. Make generator prime mover operational as designed. 46 CFR 182.100

A visual indicator must be provided at the operating station to indicate when any automatic bilge pump is operating. Bilge pump indicator at operating station not existent. Provide visual indicator for automatic bilge pump at operating station. 46 CFR 182.530(c)

Each vessel that operates on the high seas, or that operates beyond three miles from the coastline of the Great Lakes, must have on board a FCC Type Accepted Category 1, 406 MHz EPIRB, installed to automatically float free and activate. EPIRB battery was expired. Replace with new battery or new EPIRB; restricted to operate within three miles from the coastline until rectified.

The person in command of a documented vessel must have on board that vessel the original Certificate of Documentation currently in effect for that vessel. Vessel only had a copy of the COD on board. Obtain original COD from the National Vessel Documentation Center and maintain on board.

Each ring life buoy on a vessel must be approved in accordance with 160.050 in sub chapter Q. Ring life buoy's retro-reflective tape was worn and required replacement. Replace with new retro-reflective tape IAW regs.

Life floats installation and condition on board vessels must meet approval under series 160.027. Retro-reflective tape required replacement and weak link was missing. Provide new retro-reflective tape and install new weak link.

The number and identification of the items stowed inside, and their sizes, must be marked in clearly legible letters and numbers on each container for life jackets. Letters and numbers must be at least 2 inches high. Marking was faded and inaccurate in forward cabin. Replace marking identification in forward cabin.

(02) sewage valves were missing lock. Secure both overboard sewage valves in accordance with regulations. (i.e. closing each valve leading to an overboard discharge and removing the handle; padlocking each valve leading to an overboard discharge in the closed position; or using a non-releasable wire-tie to hold each valve leading to an overboard discharge in the closed position).

An EAP training program must be conducted for the employer's crewmembers and supervisory personnel. The training program must include at least the following elements: the effects and consequences of drug and alcohol use on personal health, safety, and work environment; the manifestations and behavioral cues that may indicate drug and alcohol use and abuse; and documentation of training given to crewmembers and Supervisory personnel must receive at least 60 minutes of training. Provide proof of completed EAP training for all personnel.

Vessel COI authorizes carriage of 23 total passengers. Due to recent engine repower, vessel has experienced an aggregate weigh change of 1,605 lbs. Additionally, vessel has completed electrical rewiring overhaul of the entire vessel; MSC approval pends. Moreover, special consideration for stability pending w/Sec Hono OCMI. In the interim, vessel is required to maintain passenger decrease count by 9 passengers, limiting the total number of passengers to 14. Upon determination of special consideration, vessel may need to pursue Incline Stability test for approval.

The cognizant OCMI may perform operational tests to determine whether the vessel has adequate stability and satisfactory handling characteristics under sail for protected waters or partially protected waters. Vessel underwent re-power and is required to pass SST prior to carriage of passengers. 46 CFR 178.325(c)

Currently, vessel is authorized to carry 23 total passengers. Due to recent engine repower, vessel has experienced an aggregate weight change of 1,605 lbs. Vessel has (01) outstanding deficiency requiring vessel to update DC and AC one line electrical drawing/plans for approval to MSC and complete installation prior to 25 May 2022. In the interim, vessel is required to decrease passenger count by 9 passengers, limiting the total number of passengers to 14. Upon completion of electrical deficiency, but NLT 25 May 2022, vessel must conduct a Simplified Stability Test iaw 46 CFR 178.330 to identify new maximum for total persons allowed onboard due to recent weight changes.

A pre-engineered fixed gas fire extinguishing system must be installed in accordance with the manufacturer's instructions. Fireboy system is missing appropriate YANMAR wire connection; FEDEX part delayed. Upon receipt, install part and prove satisfactory operational test of pre-engineered fixed gas fire extinguishing system.

New batteries and battery charger installed. Also inverter installed with unapproved interlock. Numerous changes to electrical load, conductors, and circuit protection. Provide updated vessel DC and AC one line electrical drawings/plans for approval to MSC and complete installion prior to 25 May 2022. 46 CFR 177.202

As a result of the equipment failure onboard the ISLAND STAR on September 5, 2021, the vessel's engine has been identified as inoperable. The vessel must identify the engine issue and make appropriate repairs to the satisfaction of the attending USCG marine inspector prior to passenger operations.

Pre-engineered fixed fire fighting override not functioning as designed. Exhaust blower/fan also shuts down when circuit is opened but this blower/fan prevents main engine from starting in override. Both 1st and 2nd captains are aware of this situation and will open blower/fan breaker to allow for override. Make repairs to electrical circuit to ensure proper operation of pre-engineered fixed fire fighting system. 46 CFR 181.420

Numerous dead end wires throughout vessel. Remove dead end wire at navigation station, engine room, main electrical panel. 46 CFR 183.200

02 permanent electronic devices plugged in. Make permanent installation and hard wire devices with proper circuit protection. 46 CFR 183.200

Circuit breaker at the switchboard must be provided and must be interlocked with vessels power sources so that shore power and the vessels power sources may not be operated simultaneously. Inverter has unapproved interlock to ensure shore power and inverter not operational at same time. Provide approved interlock. 46 CFR 183.390

New batteries and battery charger installed. Also inverter installed with unapproved interlock. Numerous changes to electrical load, conductors, and circuit protection. Provide updated vessel DC and AC one line electrical drawings/plans for approval. 46 CFR 177.202

Sewage overboard valve not locked. Provide lock for sewage overboard valve. 33 CFR 159.57

Emergency exit marking missing on outside of hatch. Both sides of hatch must be marked. Provide marking on on exterior side of emergency escape hatches. 46 CFR 185.606

Fuel fill hose missing ground wire from deck fill to tank. When flexible hose is non-conductor of electricity, the metallic sections of the fill pipe separated thereby must be joined by a conductor for protection against generation of a static charge while filling with fuel. Provide conductor from fill piece to tank. 46 CFR 182.445(g)

Main engine leaking coolant. Temporary repair has been made. Make permanent repairs to coolant system to prevent leaking. 46 CFR 182.420

Shutdown solenoid on main engine was sticking preventing engine from starting without manual activation. Manual hand activation of rod required to start engine. Replace solenoid to make engine start as designed.

Company drug policy/statement and hot line not on board or posted on board vessel. Provide policy/statement and post hot line.

Sail area exceeds limit of 1056sqft per Stability Letter. Vessel is limited to 1056sqft of total sail. Provide correct sails or prove compliance of greater sail area in accordance with 46 CFR Part 178 and 177.330.

Sail and Rigging Preventative Maintenance Plan was unsatisfactory. Provide effective PMP in accordance with 46 CFR 176.840.

Vessel EPIRB Battery is expired.

Retro tape was worn beyond serviceable condition.
