10101: In all ships, where the distance from sea level to the point of access to, or egress from, the ship exceeds 9 meters, and when it is intended to embark and disempark pilots by means of the accomodation ladder, or other equally safe and convenient means in conjunction with a pilot ladder, the ship shall carry such equipment on each side, unless the equipment is capable of being transferred for use on either side. During the vessels outbound transit, the cables supporting the port accommodation ladder failed causing the ladder to become inoperable. SOLAS (20) V/23.3.2

The safety management system shall be maintained in accordance with the provisions of the International Safety Management Code. The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be established by the company. In meeting these requirements, the company should ensure that appropriate corrective action is taken. PSCO observed 17 lashing bridges and attached cargo securing devices that were significantly worn or completely deteriorated, and cracks were visible. The lashing plates were severely rusted which rendered them ineffective and unsafe for cargo securing. A condition of class was issued on 20NOV21, prior to vessel’s dry dock from 24DEC21 - 01FEB22. Class survey was conducted on 31JAN22 after dry dock and extension was issued for the outstanding condition of class not rectified during dry dock. PSCO was told parts were ordered in February but have not yet been received. Vessel's SMS (III.2.4.8) states that the lashing equipment must be checked before usage for condition/damages. In addition, ensure regular maintenance of lashing material by de-rusting as necessary and parts replaced must be a maker original part and replaced strictly in accordance with maker recommendations. Class made the determination during dry dock period that the above equipment was approved for continued use, contradicting the vessel's SMS. Based on the equipment of the vessel as substantiated by the objective evidence cited above, an external verification is recommended to ensure ship is compliant with cargo securing procedures and equipment. 74 SOLAS (20) IX/5 ISM CODE 10.1 & ISM CODE 10.2.3

In meeting these requirements, the company should ensure that appropriate corrective action is taken. Condition of Class issued on 11MAR22 for inoperable switchboard for emergency shutdown of fuel pumps, cargo hold fans and accommodation fans being out of order with due date of 08APR22, with no extension given. In the event of an emergency, systems can only be shut down manually at breaker by qualified duty engineer. Unavailability of emergency shutdowns of critical systems significantly reduces the vessel’s ability to mitigate the potential spread of fire onboard, thus increasing the risk of a hazardous condition to the vessel and crew. Recommend thorough ISM audit. 74 SOLAS (20) IX/5 ISM CODE 10.1 & ISM CODE 10.2.3
