Every steam pipe and every fitting connected thereto through which steam may pass shall be so designed, constructed and installed as to withstand the maximum working stress to which it may be subjected. PSCO observed a steam pipe leaking in the starboard side passage way near the Pilot Station.

The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be established by the company. The vessel's safety management system states that emergency lighting must be tested and confirmed fully operational by the electrician monthly. Work order dated 10/13/2021 recorded emergency lights were checked for proper operation and busted lamps were replaced. PSCO observed in excess of 18 emergency light bulbs extinguished and cracked housing. Due to the objective evidence in the above deficiencies, the vessel is not in substantial compliance with relevant conventions. The Captain of the Port questions the adequacy and overall implementation of the vessel's SMS under the ISM Code. An internal audit is required within 30 days to determine whether the ship is operating in accordance with the ISM Code. Provide a copy of the audit report to USCG when completed.
