The officer in charge of a navigational watch must conduct a navigation assessment for the intended route and operations prior to getting underway. The navigation assessment must incorporate the requirements of pilothouse resource management of § 140.640, assess operational risks, and anticipate and manage workload demands. Ensure that other members of the navigational watch have a working knowledge of the navigation assessment required by § 140.635, and understand the chain of command, the decision-making process, and the fact that information sharing is critical to the safety of the vessel. Identified logbook had a section dedicated to navigation assessment, but vessel was not documenting or conducting assessment. Educated and cleared on the spot.

Stations in the maritime service must be licensed by the FCC. Vessel is required to maintain a Bridge-to-Bridge radiotelephony certificate. Provide certificate to the attending MI. 47 CFR 80.59(a)(2) 47 CFR 80.13 47 CFR 80.1001

Each towing vessel operating in oceans and coastwise service must have a line throwing appliance approved under 46 CFR 160.040, which requires 4 separate charges. The vessel only had one approved line throwing appliance on board with no spare charges. Obtain 3 more line throwing appliances or replacement charges/lines. 46 CFR 141.385 46 CFR 160.040

A towing vessel greater than 26 feet must carry at least two lifebuoys with lights. The lifebuoys on board were missing grab lines, and only one lifebuoy had a light. Replace or repair both lifebuoys and outfit both with approved lights. 46 CFR 141.360(c)(2)

A towing vessel that is: certificated for coastwise whose contract was executed prior to August 27, 2003, must be equipped with either an approved 160-B semi portable fire-extinguishing system to protect the engine room or a fixed fire-extinguishing system installed to protect the engine room. The currently installed 160-B (B-V) is installed inside the engine room where crew would be unlikely to safely access the extinguisher in case of a fire. Within 30 days, submit proposed plans for a new arrangement that limits potential crew exposure to engine room fire when operating the semi-portable extinguisher. 46 CFR 142.315(a)(2)

Pressure vessels must be externally examined annually. Relief valves must be tested twice every 5 years with no more than 3 years elapsing since last test. Relief valves on both port and stbd air receiver were painted over and were not tested at the time of the inspection. Replace relief valves and provide bench test report showing new valves were tested for MAWP of 250 psi or lower based on operational use. 46 CFR 143.300(b) 46 CFR 143.245

All pressure vessels must have the MAWP indicated by a stamp, nameplate, or other means visible to the crew. The stbd air receiver was marked with a dataplate, but no marking was found to indicate MAWP on the port air receiver. Produce a dataplate or other means of marking the port air receiver with MAWP. 46 CFR 143.300(c)

All pressure vessels must have the maximum allowable working pressure (MAWP) indicated by a stamp, nameplate, or other means visible to the crew. If MAWP placards cannot be located or verified by original manufacturer on both pressure vessels located in the engine compartment, they will need to be assessed by a third party to determine actual MAWP. Else, both tanks will need to be replaced with tanks that have an accurate MAWP placard. 705 (c) (At next scheduled drydock)(May 2023)
