The officer in charge of a navigational watch must conduct a navigation assessment for the intended route and operations prior to getting underway. The navigation assessment must incorporate the requirements of pilothouse resource management of § 140.640, assess operational risks, and anticipate and manage workload demands.
Ensure that other members of the navigational watch have a working knowledge of the navigation assessment required by § 140.635, and understand the chain of command, the decision-making process, and the fact that information sharing is critical to the safety of the vessel.
Identified logbook had a section dedicated to navigation assessment, but vessel was not documenting or conducting
assessment. Educated and cleared on the spot.

A towing vessel that is: certificated for coastwise whose contract was executed prior to August 27, 2003, must be equipped with either an approved 160-B semi portable fire-extinguishing system to protect the engine room or a fixed fire-extinguishing system installed to protect the engine room. The currently installed 160-B (B-V) is installed inside the engine room where crew would be unlikely to safely access the extinguisher in case of a fire. Within 30 days, submit proposed plans for a new arrangement that limits potential crew exposure to engine room fire when operating the semi-portable extinguisher.

Relief valves on both port and stbd air receiver were painted over and were not tested at the time of the inspection. Replace relief valves and provide bench test report showing new valves were tested for MAWP of 250 psi or lower based on operational use.
