The company should establish procedures to ensure that the ship is maintained in accordance with the provisions of the relevant rules and regulations established by the company. The vessel is not compliant with their SMS Engine Room Housekeeping Procedures. Deficiencies 02 and 03 provide objective evidence of a serious failure of the implementation of the ISM Code. The purpose of this regulation is to prevent the ignition of combustible materials or flammable liquids. For this purpose, the following functional requirements shall be met: means shall be provided to control leaks of flammable liquids. PSCO observed active leaks from high pressure fuel injector on number 6 cylinder. PSCOs also identified oil soaked rags on top of the main engine, within steering gear room, around the purifier space and around the auxiliary engines. 74SOLAS(04) II-2/ 4.1.1 30ac

Resolution: Received and reviewed class report. Fuel injector was found to be leaking due to an incomplete seal. Replaced with a spare on board. Oil soaked rags found in e/r were removed and stowed in metal canisters.

Resolution: Received and reviewed class report. Cause of the hydraulic leak was due to loose steering gear packing gland bolts. Bolts were tightened.
