The following protection systems shall be kept in good order... structural protection to include fire resisting divisions and protection of openings and penetrations in these devisions. PSCO noticed penetrations in fire boundaries due to new cable run leaving a 2-3 inch openings in diameter. Insulation was also removed from bulkhead in same bulkhead in engine room. 74SOLAS(20)II-2/14.2.1.1.1

Resolution: As an interim measure the bulkhead penetration has been sealed with an insulation. The ship was advised that permanent repairs were to be completed. Flag recommended that permanent rectification prior to completion of the upcoming dry dock in july 2023.

Purpose of this regulation is to present the igniton of flammable liquids. The following functional requirements shall be met: means shall be provided to control leaks of flammable liquids to control accumulation of repairs. Multiple containers of flammable dye was found on deck in a plastic container in the hazardous area. Minimum spillage was noticed in container. No MSDS in area was found by PSCO either.
