Vessel has 5 cracked welds at framing, and is missing several hatch gaskets. Repair welds and make all hatches watertight.

Cargo tanks in which Grade D or E liquids only are to be transported shall be fitted with gooseneck vents and flame screens. The flame screens on the P1, P2, and S1 were insufficient and shall be repaired to the satisfaction of the attending USCG Marine Inspector.

Each cargo tank of a tank barge must have a high-level indicating device which indicates visually the level of liquid in the cargo tank when the liquid level is within a range of 1 meter to the top of the tank. The P1 meter stick failed to meet the one meter requirement. All repairs shall be to the satisfaction of the attending USCG Marine Inspector.

All tank vessels are subject to the regulations contained in subchapter J. The vessel had an extension chord plugged into an outlet below the switchboard, crossing into a hazardous zone, breaking the intrinsically safe boundary. The extension chord shall be removed to the satisfaction of the attending USCG Marine Inspector.

Cargo discharge piping of all tank vessels shall be tested at least once each year for tightness, at the maximum working pressure. The cargo header test date, cargo hose test date, and cargo gauge testing was April 12 2022, therefore the barge failed to meet the requirements. The cargo header test, cargo piping tests, and cargo pressure gauge test shall be completed for the year 2023 and the vessel shall notify the attending USCG Marine Inspector of completion.
