Pilot ladder was rigged with shackles holding ladder at the step, which is not a point as strong as the side ropes. The platform at the bottom of the accommodation ladder not level as rigged. This is not a compliant arrangement as per SOLAS V/23 and Resolution A 1045(27).

On ships carrying flammable or Toxic products a water spray system for cooling, fire prevention and crew protection should be installed to cover cargo tank domes and cargo manifolds. Deck water spray did not provide adequate coverage over tank dome #2 Starboard or either port or starboard manifolds.
