TO PREVENT THE IGNITION OF FLAMMABLE LIQUIDS THE FOLLOWING FUNCTIONAL REQUIREMENTS SHOULD BE MET: MEANS SHALL BE PROVIDED TO CONTROL LEAKS OF FLAMMABLE LIQUIDS. A LEAK OF FLAMMABLE LIQUID WAS OBSERVED FROM PUMP #3, LOCATED IN THE PUMP ROOM. PRIOR TO CONDUCTING CARGO OPERATIONS AT NEXT U.S. PORT (BOSTON), TO THE SATISFACTION OF CLASS. MONITOR SPACE WHILE CONDUCTING CARGO OPERATIONS FOR ANY CARGO LEAKAGE IN CARGO PUMP ROOM. SOLAS 74 (2009 CONS. ED.) II-2/4.1.1

The arrangements for the distribution and utilization of oil used in pressure lubrication systems shall be such as to ensure the safety of the ship and persons on board. PSCO observed that the Main Engine #2 cylinder exhaust valve lubricating oil was actively and excessively leaking that makes it a fire hazard. Conduct permanent repairs to the satisfaction of the Certificate issuing authority. SOLAS 74(am) II-2/4.2.3.1

Resolved 12 December 2020
Resolution: Received a Class report from NKK. Crew overhauled the Main Engine #2 cylinder exhaust valve and replaced the gaskets and seals with spares. Tested Main Engine and was found in good working order with no active leaking of lube oil. Cleared deficiency.

PSCO observed epoxy repairs on mulitple inlets for main diesel engine heat exchangers and local thermometer on #2 main diesel heat exchanger. Request clarification from certificate issuing authority to the continied acceptance of the non-class approved temporary repair. Conduct repairs by 1200, 18 MAR 2018.
