All life-saving appliances and arrangements shall comply with the applicable requirements of the Code. The propeller shafting shall be so arranged that the propeller can be disengaged from the engine. Provision shall be made for ahead and astern propulsion of the lifeboat. Lifeboat 18 was incapable of astern propulsion. 74 SOLAS 20 III/34 LSA Code 4.4.6.5

Furniture in stairway enclosures shall be limited to seating. It shall be fixed, limited to six seats on each deck in each stairway enclosure, be of restricted fire risk determined in accordance with the Fire Test Procedures Code, and shall not restrict the passenger escape route. The Administration may permit additional seating in the main reception area within a stairway enclosure if it is fixed, non-combustible and does not restrict the passenger escape route. Furniture shall not be permitted in passenger and crew corridors forming escape routes in cabin areas. In addition to the above, lockers of non-combustible material, providing storage for non-hazardous safety equipment required by these regulations, may be permitted. Drinking water dispensers and ice cube machines may be permitted in corridors provided thye are fixed and do not restrict the width of the escape routes. This applies as well to decorative flower or plant arrangements, statues or others objects of art such as paintings and tapestries in corridors and stairways. Several portable chairs were found in multiple stairway landings and elevator landings. 74 SOLAS 20 II-2/5.3.3

Fire-fighting systems and appliances shall be kept in good working order and readily available for immediate use. Fire stations 9.I.02 and 9.III.01 doors were found stuck closed. Fire station 10.V.9 was found unable to be closed while fire hose was connected. Fire station 7.11.05 had multiple trash bags stored inside. Fire station in Deck 9 Lido Galley was blocked by dish trolley. 74 SOLAS 20 II-2/14.2.1.2

Electrical installations shall be such that: the safety of passengers, crew and ship from electrical hazards will be ensured. Deck 12 AC station 12304 electrical cabinet was open, with too much equipment inside to close, and wires found on the deck. Deck 6 frame 282 had a low voltage panel missing an electrical cover. 74 SOLAS 04 II-1/40.1.3

In addition to the requirements contained elsewhere in the present regulations, ships shall be designed, constructed and maintained in compliance with the structural, mechanical and electrical requirements of a classification society which is recognized by the Administration in accordance with the provisions of regulation XI-1/1, or with applicable national standards of the Administration which provide an equivalent level of safety. Multiple cracks were found in crew stairwell near FSD 07.4.22. Significant corrosion resulting in a hole was found in A class bulkhead by FSD 10.2.38. Piping was found near FSD 07.01.56 from category 10 to category 3 through A Class bulkhead with gap found between pipe and bulkhead. 74 SOLAS 20 II-1/3-1

The purpose of this regulation is to contain fire in the space of origin. For this purpose, the following functional requirements shall be met: the ship shall be subdivided by thermal and structural boundaries. Insulation as found missing in door frame connecting passenger to crew area in vicinity of midship elevator outside stateroom 6118. 74 SOLAS 20 II-2/9.1.1

Where the contents and use of a space are such that there is a doubt as to its classification for the purpose of this regulation, or where it is possible to assign two or more classifications to a space, it shall be treated as a space within the relevant category having the most stringent boundary requirements. Cardboxes, foam rolls, and flammable glue was found inside category 10 space, AC room 5604 with FSD 5.6.5. 74 SOLAS 20 II-2/9.2.2.3.2.2

74 SOLAS 20 II-2/9.2.2.3.2.2, 10(C)- Spaces are classified according to their fire risk as shown in categories (1) to (14). Pool equipment room is a category (10) space where storage of combustibles is prohibited, PSCO observed storage of cardboard boxes and other combustible materials stored within this space.

74 SOLAS 20-22/13.3.2.3. 10(C)- Within the perimeter of such stairway enclosures, only public toilets, lockers of non-combustibles material providing storage for non-hazardous safety equipment and open information counters are permitted. PSCO observed hand sanitation stations and general information signs placed within category (2) stairway enclosures throughout the ship.

74 SOLAS 20 II-2/4.4.2 10(C)- Waste receptacles shall be constructed of non-combustible materials with no openings in the sides or bottom. PSCO observed a plastic waste receptacle with combustible material in one of the ship's galleys.

Rescue boat launching arrangements shall be such that the rescue boat can be boarded and launched in the shortest possible time. The hydrostatic release mechanism on rescue boat/life boat #1 was not functioning properly and required manual release. 74 SOLAS 20 III/17.1 (16ac)

The load line ring, lines and letters shall be painted white or yellow on a dark background or in black on a light background. They shall also be permanently marked. The deck line on the port side of the vessel was not painted with a contrasting color. ICLL 6 (50ac)

All survival craft required for the abandonment by the total number of persons on board shall be capable of being launched with their full complement of persons and equipment with in a period of 30 min from the time the abandon ship signal is given after all person have been assembled, with lifejackets donned. The team assigned to deploy life rafts on the port side of the vessel was unfamiliar with the launching procedures and could not meet the time requirements. 74 SOLAS 20 III/21.1.3 (10c)

Fire doors in main vertical zone bulkheads, galley boundaries and stairway enclosures shall be self-closing and be capable of closing with an angle of inclination of up to 3.5° opposing closure. The fire door on deck 8 outside of cabin 8180 would not close. 74 SOLAS 20 II-2/9.4.1.1.5.1 (10c)

All life-saving appliances and arrangements shall comply with the applicable requirements of the Code. Except as otherwise stated, the normal equipment of every life boat shall consist of a food ration totaling no less than 10,000 kJ for each person the lifeboat is permitted to accommodate. One ration on life boat #3 was torn open. 74 SOLAS III/34 LSA Code 4.4.8.12

07105 - Fire doors/ openings in fire-resisting divisions - The construction of doors in door frames in "A" class divisions with the means of securing of them when closed, shall provide resistance to fire as well as to the passage of smoke and flame. Fire screen door 03.2.63 did not close. SOLAS 09 II-2/9.4.1.1.2

07105 - Fire doors/openings in fire-resisting divisions - Where ventilation ducts with a free cross sectional area exceeding 0.075m2 shall be fitted with fire dampers... provided with an indicator which shows whether the damper is open or closed. Visual indication for automatic damper C0/310137ft found in fault status. SOALS 09 II-2/9.4.1.1.2

0715-fire detection-As required ships automatic sprinkler system shall include means for giving a visual and audible alarm signal automatically at one or more indicating units whenever any sprinkler comes into operation. Such alarm systems shall be such as to indicate if any fault occurs in the system. Such units shall indicate in which section served by the system a fire has occurred. While testing section valves in various locations around the vessel it was discovered that section 22 was indicated as the active location, no matter what section was tested. This does not indicate the actual location that was activated and may cause confusion when determining the fire location. Provide a class report attesting to the proper operation of the automatic sprinkler alarms and indicators.- SOLAS (2014) II-2 Reg 10.6.1.1/ FSS code Ch. 8.2.5.2.1

Each survival craft shall be stowed in a state of continuous readiness so that two crew members can carry out preparations for embarkation and launching in less than 5 minutes. The launching appliance for the following lifeboats were inoperable. Lifeboats 6, 8, 9, and 18. Attending inspectors witnessed proper operation of all portside lifeboat launching appliances, and spot checked launching appliance on lifeboat 9 on stbd side. Vessel to notify Flag State of corrective action taken.

Escape routes shall be maintained in a safe condition, clear of all obstacles. Oil drums and other obstacles are located in 1-95 that obstruct egress. IAW SOLAS (09) II-2/13.1.2
