The oil discharge monitoring and control system shall be designed and installed in compliance with the guidelines and specifications for oil discharge monitoring and control systems for oil tankers developed by the Organization. USCG Sector Houston-Galveston received a report of a flow fault error on M/T OVERSEAS CHINOOK during their monthly ODME testing. Enact repairs to the satisfaction of the RO. MARPOL ANNEX I/31.3

Vessel reported 16DEC2024: experienced unexpected Main Engine speed control issues. At no load, the engine will hunt between 70-100 rpm. At dead slow the engine will hunt 5-8 rpm above or below the speed table setting; affects vessel safety construction certificate. Therefore, proper repairs shall be conducted and approved by vessel's RO. After repairs and testing have been approved by RO (ABS), applicable report shall be sent to USCG SECMOB for clearance to enter port.

Before a ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and readily for immediate use. Rescue boat davit control box had a broken ground cable and the armor sheathing broken exposing wires. Referred to Class.

Clear instructions to be followed in the event of an emergency shall be provided for every person on board. Lifejackets and immersion suits did not have donning instructions up forward by 6 person life-raft. Instructions were printed, laminated and posted.
