Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. Free fall lifeboat hydraulic steering not fully operational.

MARPOL Annex 1/6.4.2 After any survey has been completed, no change shall be made in the structure, equipment, system, fittings, arrangements or material covered by the survey, without the sanction of the Administration, except the direct replacement of such equipment and fittings. Ship's Drawing No. SP25-AB1PH-01 DOES NOT INDICATE A 1/4in ball valve found on the sample discharge line on the oil content meter. Operating procedures do not indicate the position of the valve when systems is running. PSCO witnessed a test of the system with the valve in the closed position (creating no sample water flow to OCM) and observed the system continuously operating without a true sample of effluent with an adequate pressure and flow.
