Irrespective of build date, a vessel to which this sub chapter applies must meet 46 CFR 181.405, 181.450, 181.500, 185.364 and 185.420(b), if it is not a ferry and it (2) is operating on a Coast wise or Oceans route. Vessel does not have an interconnected smoke detection system in the engine room. VSL is restricted to a Limited Coast Wise route until deficiency is rectified.

A vessel must be fitted with a satisfactory arrangement for draining any watertight compartment. Bilge suction in the accommodation space had a continuous leak while testing bilge suction overboard.

At each initial and subsequent inspection for certification of a vessel the owner or managing operator shall have the vessel ready for inspections of the hull structure to include the following: Inspection of all accessible parts of the exterior and interior of the hull, and the watertight bulkheads. MI noted cracked and missing welds in fore peak on forward port bulkhead one frame down from deck line and along starboard side forward of the collision bulkhead one frame down from deck line. Repair welds prior to credit drydock.

A bilge pipe piercing the collision bulkhead must be fitted with a screw-down valve located on the forward side of the collision bulkhead and operable from the weather deck, or, if it is readily accessible under service conditions, a screw-down valve without a reach rod may be fitted to the bilge line on the after side of the collision bulkhead. Valve fitted on bilge line aft of the collision bulkhead must be screw-down.

Vapor barriers must be provided where insulation of any type is used in spaces where flammable and combustible liquids or vapors are present, such as machinery spaces and paint lockers. Wire penetrations in vapor barrier of aft engine room bulkhead are not vapor tight.

Vessel reequired self priming bilge pump to dewater spaces. During operational test a water stream was observed due to a hole in bilge piping.

Remote fuel shutoff valve required to stop flow of fuel to engine. During operation test port fuel shut off did not actuate.Corrected on the spot.

At each inspection for certification and every other vessel inspection the bilges and other spaces may be examined to see that there is no excessive accumulation of oil, trash, debris, that might create a fire hazard, clog bilge pumping systems, or block emergency escapes. Vessel had oil in the bilges in the Engine Room and Steering Gear room.

Must have interconnected detection and alarm system. This must consist of multiple-station smoke detectors listed by an NRTL, or independent laboratory accepted by the Commandant, as meeting UL 217, be installed such that the actuation of alarm in one area results in both audible and visual alarms in all required areas protected by the interconnected detection and alarm system, contain an independent power source, and alarm on low power. A fire detection and alarm system installed in accordance with 46 CFR part 76 would satisfy the requirements of the section. AVA FOSS was noted on Sector LALB as having not provided the fixed detection. Vessel queried, and a system has been installed, USCG Sector LALB will verify compliance during May 2023 Drydock.

All escape hatches and other emergency exits used as means of escape must be marked on both sides in clearly legible letters at least 50 millimeters (2 inches) high: “EMERGENCY EXIT, KEEP CLEAR”, unless such markings are deemed unnecessary by the cognizant [name withheld] escape hatch was not properly labeled.

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date tide tables.

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked “First-Aid Kit”. A first-aid kit must be easily visible and readily available to the crew. Medicine in medical kit was expired.

Stowed so that when the vessel sinks the survival craft floats free and, if inflatable, inflates automatically. Liferafts were arranged in a manner that would prevent them from being released by the hydrostatic release mechanism.

Except as noted in paragraph (c) of this section, stations in the maritime service must be licensed by the FCC either individually or by fleet. Vessel's FCC cerificate was expired.

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. Vessel experienced a loss of propulsion to the port engine while transiting underway in King Harbor, Redondo Beach. Vessel operator must demonstrate proper operation of the port engine to the satisfaction of the attending marine inspector. 46 CFR 182.100

Each portable fire extinguisher on a vessel must be of an approved type. Vessel missing one fire extinguisher that meets the UL 2-A performance classification standard.

Vertical or horizontal surfaces within 3 feet of cooking appliances must have an ASTM E-84 Flame spread rating of not more than 75. Vessel crew using unapproved installation of toaster oven.

Alterations and repairs to inspected vessels must be done - 1)Under the direction of the OCMI; and 2)Except as provided in paragraph (b) of this section, in accordance with the regulations in this subchapter, to the extent practicable. Vessel is restricted from carrying cargo until new stability letter is issued addressing vessel alterations and repairs made at drydock (fendering alterations).

FOUND THAT THE VESSEL WAS MISSING THE DAPI CREW ROSTER. PROVIDE PROOF OF CREW MEMBERS ENROLLED IN DAPI.

Conduct servicing of the fixed fire extinguishing system. USCG to be present during testing.

Provide instructions for the operation of the fixed gas fire extinguishing system at each station, local/remote.

Replace fire pump outlet pressure gauge.

Prove proper operation of port MDE and port MDE water pump prior to carrying passengers. 46 CFR 182.420(a)

Prove proper operation of forward ventillation closures in engine room

Prove proper operation of 24 volt battery system

Prove proper operation of ships horn

Provide alcohol test strips for DAPI. Vessel limited to 2hrs from test facility until provided on board.

PROVE PROPER OPERATION OF BOTH PORT AND STARBOARD ENGINES PRIOR TO CARRY PASSENGERS.

Prove proper operation of starboard main engine.

Obtain a revised Stability Letter from Marine [name withheld] indicating fixed ballast (1458 lbs.) on starboard side between fames #1 and #2.

Prior to carrying passengers, vessel master and crew must perform emergency drills (including but not limited to man overboard, abandon ship, and fire drills) to the satisfaction of attending USCG Marine Inspector.
