Resolution: Corrected on the spot by removing the bolts blocking the quick closing valves and providing dcumentation that they had conducted testing of the valves within the past 24 hours.

The condition of the ship and its equipment shall be maintained to conform with the provisions of he present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. Master reported damage to the vessel which occurred while mooring at the port. PSCO attended the vessel and observed a gouge in the port side hull on the outside of the ballast tank approximately 15 feet(4.6 meters) in length with a 1.5 inch (3.8 centimeters) inset resulting from an exposed shackle due to lack of fendering on the port.

Resolution: DNV GL Class Survey indicated No further action or repairs required as the indent is considered within acceptable tolerances and of no significance.
