The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations. The technical and operational nature of deficiencies 2,3, and 4 provide objective evidence of a serious failure, or lack of effectiveness, of the implementation of the ISM Code. A safety management audit carried out by the Administration or the RO, with a corresponding report of findings satisfactory to the OCMI must be submitted prior to clearance of this deficiency within 90 days. 74SOLAS(24) IX/3.1 ISM Code 10.1

The machinery, boilers and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as reduce to a minimum any danger to persons on board, due regard being paid to moving parts, hot surfaces and other hazards. When the fire-extinguishing medium is stored outside a protected space, it shall be stored in a room which is located behind the forward collision bulkhead. Any entrance to such space shall be preferably be from an open deck…Spaces which are located below deck or spaces where access from the open deck is not provided shall be fitted with a mechanical ventilation system designed to take exhaust air from the bottom of the space and shall be sized to provide at least 6 air changes per hour. Access doors shall open outwards, and bulkheads and decks, including doors and other means of closing any opening therein, which form the boundaries between such rooms and adjacent enclosed spaces shall be gastight. PSCO observed that the vessel had a mechanical ventilation system installed for the CO2 storage room, but the ventilation was not operational.
