THE PURPOSE OF THIS REGULATION IS TO PREVENT THE IGNITION OF COMBUSTIBLE MATERIALS OR FLAMMABLE LIQUIDS. FOR THIS PURPOSE, THE FOLLOWING FUNCTIONAL REQUIREMENTS SHALL BE MET: MEANS SHALL BE PROVIDED TO CONTROL LEAKS OF FLAMMABLE LIQUIDS. PSCO OBSERVED EXCESSIVE OIL ACCUMULATION IN WAY OF JACKING GEAR FOR THE MAIN ENGINE, MAIN ENGINE PIPING BELOW FUEL INJECTORS, BELOW COMMON RAIL AND ASSOCIATED LAGGING, FUEL PURIFIER INLETS, AND THE LOWEST LEVEL FORWARD BUNKER STATION VALVE IN THE ENGINE ROOM. SOLAS (09) II-2/4.1.1

ALL STEERING GEAR COMPONENTS AND THE RUDDER STOCK SHALL BE OF SOUND AND RELIABLE CONSTRUCTION TO THE SATISFACTION OF THE ADMINISTRATION. THE #1 STEERING PUMP WAS OBSERVEED TO BE DRIFTING 10 DEGREES PORT EVERY MINUTE DURING THE LOCAL STEERING TEST AND FROM THE BRIDGE STEERING UNDER NON-FOLLOW UP.

THE RING, LINES AND LETTERS SHALL BE PAINTED IN WHITE OR YELLOW ON A DARK GROUND OR IN BLACK ON A LIGHT GROUND. THE MARKS SHALL BE PLAINLY VISIBLE AND, IF NECESSARY, SPECIAL ARRANGEMENTS SHALL BE MADE FOR THIS PURPOSE. THE DECK LINE ON BOTH PORT AND STBD SIDES OF THE SHIP ARE PAINTED BLACK ON A BLACK BACKGROUND AND ARE NOT CLEARLY VISIBLE.

CYLINDERS MUST ALWAYS BE SECURED AND, WHEN NOT IN USE, THEY MUST BE STOWED IN A RACK IN AN UPRIGHT POSITION, WITH THE VALVE PROTECTION CAP IN PLACE. OXYGEN AND ACETYLENE BOTTLES NOT IN USE DID NOT HAVE PROTECTION CAPS AND THE CREW DID NOT HAVE SPARES AVAILABLE.

To prevent the ignition of combustible materials or flammable liquids, means shall be provided to control the leaks of flammable liquids. Several sections of high pressure fuel piping insulation and piping were oil soaked and soiled. The crew wrapped rags around a section of actively leaking piping, when the oil soaked rag was replaced with a clean rag, the clean rag immediately became soaked with oil. Oil would continuously drip from this rag as well as the oil soaked insulation onto the platform and into the bilges. 74SOLAS(04)II-2/4.1.1

To contain a fire in the space of origin, the ship shall be subdivided by thermal insulation and structural boundaries, thermal insulation of boundaries shall have due regard to the fire risk of the space and adjacent spaces and the fire integrity of the divisions shall be maintained at openings and penetrations. The crew acknowledged that large sections of thermal insulation has been removed to facilitate installation of a scrubber two years ago but were never provided the insulation ot replace what was removed and discarded between the cargo deck and the engine room. the impacted area requires thermal insulation in order to achieve an A30 boundary. 74SOLAS(04)II-2/9.1.2

Means shall be provided to control leaks of flammable liquids. PSCO observed excessive accumulation of oil and oil soiled insulation in the purifying space. 74SOLAS(04)II-2/4.1.1

Action: 17AC, Rectify prior to departure to satisfaction of RO and Coast Guard. Each oil-fired boiler which is intended to operate without manual supervision shall have safety arangements which shut off the fuel supply and give an alarm in the case of low water level, air supply failure or flame failure. PSCO observed multiple alarms on boiler. C/E stated the vessel is having to operate manually and stand 24 hour watch. Vessel is authorized to be unmanned machinery space. SOLAS 74 A (09) II-1/32.2

Action: 17C, Rectify prior to departure to satisfaction of Coast Guard. Whenever an accident occurs to a ship or a defect is discovered, either of which affects the safety of the ship or the efficiency or completeness of its life-saving appliances or other equipment, the master or owner of the ship shall report at the earliest opportunity to the administration. Vessel could not provide notification to class of boiler alarms. -SOLAS 74 A (09)I/11(c)

1499- Other(Prop. & Aux. Machinery)- The machinery, associated piping systems and fittings shall be so installed and protected as to reduce to a minimum any danger... PSCO identified mulitiple oil leaks on machinery in the engine room: substantial fuel oil leak on the main engine #3 cylinder fuel pump barrel, a leak on #8 cylinder fuel pump barrel, #1 ship service generator in vicinity of fuel oil leakage tank return line, as well as the #3 SSDG fuel oil leakage tank return line - fitted with a 5 gallon bucket at which at the time was approximately 1/3 filled with fuel oil. 0610 - Lifeboat - Each survival craft shall be stowed fully equipped as required by the code. PSCO discovered both lifeboats were equipped with 03 parachute flares each. SOLAS74(2014CONS.) III/13.1.4 LSA CODE 4.1.5.1.10

2515 - Company Responsibility and Authority - The company should establish procedures, plans and instruction including checklists as appropriate for key shipboard operations concerning the safety of the personnel, ship and protection of the enviroment. The inventory list provided for the lifeboat requires 03 parachute flares, which is insufficient in accordance with the LSA Code. SOLAS74(2014 CONS.)IX/3.1 ISM Code Part A 7
