Creation of a crew area in the mezzanine lounge has changed a public access area that includes Passenger assembly area 2 to an employee access area. This change may conflict with certain approved plans/drawings (e.g. Fire and Safety, Lifesaving arrangement,Compliance Signage/Assembly, etc.). If this arrangement is to be retained submit a detailed plan to the OCMI for review.

A survey, either general or partial, according to the circumstances, shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel. Vessel reported abnormality with pitch controller and unable to proceed faster than 50% pitch. Vessel to repair to the satisfaction of the attending marine inspector prior to carrying passengers. 46 CFR 71.40-1

The following emergency lighting and power loads must be arranged: Illuminated signs with the word "EXIT" in red letters throughout a passenger vessel so the direction of escape to the open deck is obvious from any portion of the vessel usually accessible to passengers and crew, except machinery spaces, and except stores and similar spaces where the crew are not normally employed. Photo-luminescent "EXIT" signs throughout the vessel are marked "Replace before Oct 2025" on the manufacturer's label, and have lost their photo-luminescent properties.

The following emergency lighting and power loads must be arranged: lighting...for damage control lockers. Battery powered e-light EP201-8 in emergency squad locker on the passenger deck is not illuminating properly when tested.

In all ventilation systems, manually operated dampers or other suitable means shall be provided in accessible locations, outside the spaces served by the system, for shutting off the passage of air in the event of fire. Multiple manually operated dampers on deckhouse exterior with bolts/wing nuts and hand levers found with hardware seized, preventing proper actuation. Replace hardware to make properly usable.

Each panelboard must have a circuit directory that has the circuit designation of each circuit, a description of the load of each circuit, and the rating or setting of the overcurrent protection device for each circuit. Breaker panels P201 and P202 in pilothouse #1 and #2, respectively, have circuit 1 labeled as 15A circuit for coffee maker and circuit 3 as 20A circuit for hot/cold water dispenser. However, both breaker panels actually have a 20A circuit breaker on circuit 1 and a 15A circuit breaker on circuit 2. Determine correct breaker settings and circuit panel directory labels and correct necessary.

All general alarm bells shall be identified by red lettering at least 1/2 inch high: "GENERAL ALARM - WHEN BELLS RINGS GO TO YOUR STATION". General alarm bell in #2 end reduction gear room, near the hi-fog skid, is missing its label. Additionally, the rotating red general alarm light in the center of the #2 end reduction gear room is also missing its label.

Each elevator on a vessel must meet the requirements of ANSI A 17.1. Passenger and freight elevators displayed expired annual operating certificates in the elevator cars.

..all lifejacket containers must be marked in block capital letters with minimum quantity...size of the lifejackets stowed inside the container. Lifejacket stowage bin located in passenger assembly station on no. 1 end of mezzanine level was missing its wall-mounted placards indicating lifejackets and quantities.

Cylinders must always be secured and, when not in use, they must be stowed in a rack in an upright position, with the valve protection cap in place. Two compressed gas cylinders in the No. 1 end tank room were stowed without caps and regulators installed.

Biobore locker N0.1 end not rated UL30 or UL 1313. Provide proper storage locker or relocate flammable ships stores to an appropriate location.

Class three water tight door in the engine room does not have an audible warning signal when operated remotely. Provide controls IAW ASTM F 1197-11.5.

The prime mover of a generator set must have all accessories necessary for operation and protection of the prime mover. Block heater for the emergency generator is not operational. Provide proof of repair of inoperable block heater to marine inspector. Cite: 46CFR112.50-1(a)(1)

Periodic safety tests must be conducted on alarms and safety control for maneuvering systems. Vessel does not currently have test procedure for low lube oil on the steering sumps. Submit plans for review to incorporate this requirement to the Marine Safety Center within 60 days.

An inspection, either general or partial depending on the circumstances, shall be made whenever any important repairs or alterations are undertaken. Significant pitting in stern tubes on ends #1 & 2 requires repair/replacement and re-installation of the propulsion shafts. Repair/replace to the satisfaction of the attending marine inspector. 46 CFR 71.55-5

An inspection, either general or partial depending on the circumstances, shall be made whenever any important repairs or alterations are undertaken. Due to drydock availability, the rudders and port side keel cooler were not able to be put back on prior to leaving drydock. Reinstall rudders and keel cooler to the satisfaction of the attending marine inspector. 46 CFR 71.55-5

Submit a qualitative failure analysis to indicate automation assumptions, vessel/equipment, operating conditions, failures considered, cause and effect relationships, method of crew detection of failure and alternatives available to the crew of the ship's service and emergency switching gear. Provide a corrective action for any single point failures (non-concurrent) identified, updated and submit the required plans for approval with in 60 days. Any modifications or equipment replacement needed to effect the corrective actions shall be carried out at the vessel's next dry dock and demonstrated via design verification tests to the satisfaction of the attending Marine Inspector. Vessel reported malfunction in 2A circuit of No. 2 end steering system when transferring from transit to maneuvering mode, resulting in deviation on rudder angle indicator by 6 degrees. Although steering system is back and operational, vessel is required to replace connections on terminal strips for 2A circuit of the No. 2 end. Until then, vessel required to have both shafts engaged for steering redundancy.

A single non-concurrent failure in a control system must not prevent sustain operation of a vital system. A failure of the EDG control system resulted in the opening of the emergency bus tie breaker rendering the emergency power supply system in a dead bus condition, without means to energize emergency loads from either the main source or the emergency source. Provide an analysis and corrective action to ensure that the emergency bus tie breaker is capable of providing power to the emergency loads in the event that a failure in the EDG control system is sustained. If modifications are required they should be submitted to the USCG MSC for review and approval. 46 CFR 62.30-5(a), 112.05-1 & 5

During periodic safety testing of emergency generator, automatic transfer of emergency power load was unsuccessful. Demonstrate proper operation of automatic transfer of emergency power load IAW approved PSTP to satisfaction of an attending marine inspector. 46 CFR 112.20-15

Emergency instructions must include information on the essential actions they must take in and emergency, the location of life-jackets and donning instructions. The life jacket stowage release instructions were found to be faded and hard to read.

Complete but simple instructions for the operation of the system must be located at or near all pull boxes, stop valve controls and in the CO2 cylinder storage room. The EDG's room (and CO2 bottles location) CO2 system's instructions did not depict a clear step by step picture on how to activate the system.

Where natural ventilation is provided for spaces protected by CO2 extinguishing systems, provisions shall be made for easily and effectively closing off the ventilation. The door leading to the EDG does not seal all the way around. The space is CO2 protected and requires a complete seal.

On vessels required by 46 CFR 112.05-5a to have temporary emergency power source, the following emergency lighting and power loads must be arranged so that they can be energized from the temporary emergency power source. Including low location, stairways and public spaces. A number of emergency lights were found burnt out.

The inspection shall be such as to ensure all parts of the vessel and its equipment is in all respects satisfactory by applicable statutes and regulations. The vessel was updating its steering system an unable to provide a steering gear test. 01 outstanding deficiency (regarding this)from previous inspection remains open.

Auxiliary heating boiler space has no semi-portable fire extinguisher. Space requires 1 160-B(old B-V) extinguisher IAW Table 76.50-10(a).

Provide documentaiton for annual reviews of vessel reposnse plan. 33 CFR 155.5070(a)

The maintenance of the rescue boats wire ropes are not maintained in accordance with manufacturers instructions. See Welin Lamnie Technical Manual Section 4.5. 46 CFR 199.190(b)

All automatically or remotely controlled or monitored vital systems must be subjected to test inspections. Test procedures must be submitted and approved by the Coast Guard Rev. 0 and Rev. 1. No test procedures listed for emergency generator, low lube oil pressure, overspeed trip, and for high cooling water temperature.

Replace Hot Water heating boilers supply and return valves. They do not stop water.

Installed CO2 time delay in the emergency generator fixed CO2 system did not function as designed, delaying the release well over the designed 30 seconds. Replace and test new time delay. Until a new time delay is installed the directional control valve shall be left open as if the time delay already activated the valve, to ensure that the space can be flooded with CO2. A notice on the emergency generator room and at the CO2 controls shall inform the crew that the time delay is currently bypassed.

If a lever type control is provided, it must be installed and marked so that its movement clearly indicates both the direction of the rudders movement, and if follow up control is provided, the amount of rudder movement, current arrangement has no markings on opposing helm levers.

Each vessel must have a steering failure alarm that actuates an audible and visible alarm in the pilot house when the actual position of the rudder differs more than 5 degrees from rudder position ordered by the follow up controls. In cruising mode the rudder lever was in the port 10 degrees position and no alarm was activated

Markings in the pilothouse must not interfere with the helmsman's vision, but must be clearly visible at night, does not have illumination to read lever position.

Vessel's steering system has been found to be capable of indicating control in both pilot houses if the transfer of control button is not fully pressed in. While indication of control is illuminated at both helm stations each helm has partial to no control of steering. Vessel is authorized to continue operation provided they implement a mitigation plan. The plan shall require the master and chief mate to provide positive confirmation of a complete transfer of control by indication of the green in command light and red not in command light on either end. After positive confirmation, the in command helm shall test both bow and stern rudders to verify command prior to leaving the dock of each crossing. In addition an AB or the mate shall remain in the not in command pilot house to verify that command is never indicated or alarms activated. The mitigation plan shall remain in effect till such a time that the steering systems have been proven to be in satisfactory compliance.

Complete but simple instructions for the operation of firefighting systems must be located at each control station. CO2 and high fog remote and local operating stations were not marked. During the fire drill the crew demonstrated a lack of CO2 system operation. Provide instructions at the location of each control.

Indicate the number of lifejackets in each stowage location. Port side mezzanine was previously marked with tape that has since been removed. Mark location with accurate number of lifejackets.

03 Strobe lights emitted insufficient lighting/visual indication of general alarm in STBD FWD ENG RM (1) and in #1 RED GEAR RM (2).

Compass deviation card has not been updated since 2011.

NFU mode for Steering Gear Pumps on #1 END not indicating proper helm arrangement. In NFU mode, rudder indicates port when positioned to starboard, and starboard when positioned to port.

Steering gear not provided with power disconnect in the steering gear space.

Remote Propulsion Control in both pilot houses has no audible alarm upon failure.

Repairs to correct persistent power supply ground fault not as depicted in approved plan. Provide "as built" details that differ from approved plan for approval.

UPDATE FIRE CONTROL PLAN. SUBMIT NEW PLAN TO U.S.C.G SECTOR PUGET SOUND FOR APPROVAL. 46 CFR 71.65-5(d)

#1 Main Engine did not shut down when the E-stop button in the #1 Pilothouse was activated. Demonstrate proper function to the attending USCG Inspector.

#1 Rescue Boat requires dynamic brake test due to bridle modifications.

Crew must complete manual draft readings twice daily (beginning & end of operations). Additionally, if any unusual listing occurs crew must complete manual draft readings to ensure vessel is within correct range IAW vessel stability letter. Manual readings shall continue until system is functioning as intended.
