To ensure the safe operation of each ship in a post flag state detention environment, DPA failed to ensure that the boiler had been repaired prior to departure from lower anchorage. Due to the improperly repaired boiler, lube oil temps were identified to be 23C prior to departure. This directly led to high lube oil viscosity and the subsequent clogging of lube oil filters, low flow alarm, automated reduction of engine RMP's and associated casualty.
