Suitable portable fire-extinguishing equipment for the porduct to be carried shall be provided and kept in good operating order. Both fire-extinguishers at the port and starboard cargo maniforlds were not in proper brackets and adequately secured.

Electrical equipment, cables and wiring shall not be installed in the hazardous locations unless it conforms with the standards not inferior to those acceptable to the organization. Two sections of electrical cable, leading to BWT gauging, were found to have the outer sheating punctured. This cable condition does not meet the requirements for hazardous location in IBC Code. SOLAS 74 (14) VII 10.1 IBC Code 10.1.4

Resolution: The insulated cable was determined to be a hydraulic pipe insulated with a rubber coating, which was repaired anyway.

Tankers shall be equiped with at least one portable instrument for measuring oxygen and flammable vapor concentrations. Suitable means shall be provided for the calibration of such intruments. Crew was unable to demonstrate proper function and calibration of portable meters. SOLAS 74 (04) II-2 4.5.7.1
