The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to ship or persons on board. PSCO observed multiple Butterworth plates leaking inert gas into atmosphere, via inoperable gasket /sealant. The #2S Butterworth plate is also missing bolt designed for keeping plate secured. Inert gas is also escaping cargo tank (#2) via missing bolt. 74SOLAS(14)I/II(a) 60A/C

Resolution: All affected Butterworth plates were resealed with new sealant. Preformed leak test using soap water and tested O2 content in cargo tanks. O2 content was well below 8%.

Every ship shall be provided with a main and auxiliary steering gear. The ships #1 steering pump is not operational. PSCO witnessed the rudder, without order, swing to hard port when both steering pumps are running concurrently. Additionally, when the steering system is running continuously, for a prolonged time on steering pump/gear #1 the rudder will go to hard port without being ordered to.
