Resolution: Terminal block which transmit signal for the alarm of IG Deck Main Line High Pressure, was not making proper contact with IG Deck Main Line Pressure and O2 Content Recorder. Terminal block was secured properly and IG high pressure alarm was tested SAT.

Resolution: Aft starboard explosion proof light fixture cable gland was re-installed to maintain explosion proof rating. Forward port explosion proof light fixture was renewed with spare fixture.

PSCO observed the oil content meter history display an active status along with alarm activations for several hours on 21Apr16 and 22Apr16.No log entry was completed. See attached 5437B.
