Maintenance, testing and inspection shall be carried out based on the guidelines developed by the Organization and in a manner having due regard to ensuring the reliability of the CO2 fixed gas extinguishing system. PSCO noted that the hydrostatic testing of 10 percent of the system's extinguishing agent/cylinders and pilot cylinders were not carried out.

Audible and visual alarms shall be provided for the inert gas system high pressure condition. Crew could not demonstrate the operation of the inert gas system high gas pressure alarm in the CCR. SOLAS 74 II-2/4.5.5.3.2 and FSS Code 15.2.2.4.5.1.1

Resolution: Terminal block which transmit signal for the alarm of IG Deck Main Line High Pressure, was not making proper contact with IG Deck Main Line Pressure and O2 Content Recorder. Terminal block was secured properly and IG high pressure alarm was tested SAT.

No electrical equipment shall be installed in any space where flammable mixtures are liable to correct unless it is appropriare to the space concern. Two electric lighting fixtures located on the cargo deck are deteriorated and/or are compromized due to improper electrical connection terminations. SOLAS 74(04) II-1/45.10

Resolved 15 March 2019
Resolution: Aft starboard explosion proof light fixture cable gland was re-installed to maintain explosion proof rating. Forward port explosion proof light fixture was renewed with spare fixture.

1710-Oil Record Book- Oil tanker 150 gross tons and above is required to record in the oil record book each completed operation involving discharge overboard or disposal otherwise of bilge water. PSCO observed the oil content meter history display an active status along with alarm activations for several hours on 21Apr16 and 22Apr16.No log entry was completed. See attached 5437B.
