An embarkation ladder complying with the requirements of paragraph 6.1.6 of the Code extending in a single length from the deck to the waterline in the lightest seagoing condition under all conditions of trim of up to 10 degrees and a list of up to 20 degrees either way shall be provided at each embarkation station. The embarkation ladders (Port & Starboard) condition was found to be insufficient/unsafe due to deterioration of the line/s that form the ladder support the rungs.

For ships constructed on or after 1 July 2014, a minimum of tow tow-way portable radiotelephone apparatus for each fire party for fire-fighter's communication shall be carried on board. Those two-way portable radiotelephone apparatus shall be of an explosion-proof type or intrinsically safe. Ships constructed before 1 July 2014 shall comply with the requirements of this paragraph not later than the first survey after 1 July 2018. During the fire drill it was identified that the fire party did not have two-way portable radiotelephone apparatus available for use.

An immersion suit, complying with the requirements of section 2.3 of the Code or an anti-exposure suit complying with section 2.4 of the Code, of an appropriate size, shall be provided for every person assigned to crew the rescue boat or assigned to the marine evacuation system party. Two immersion suits were found to be deficient due to damaged seams (i.e. adhesive on seams separating). The U.S. Coast Guard has noticed a high rate of failure regarding the seams on this particular model of immersion suit.
